Yamana Gold – Responsibility – Health and safety

A significant incident reporting procedure (SIR) has been put in place at each of our mining sites. This allows us to draw conclusions from incidents that have occurred, which are then disseminated throughout the organization to prevent such incidents from recurring.

Any significant incident – avoided or occurred – is investigated in order to analyze the cause using the ICAM (Incident Cause Analysis Method). The results of these surveys are shared each month with the managers of the various mining sites and the head office, including senior management, to strengthen a culture of safety and accountability within Yamana.

Incident Cause Analysis (ICAM)

The learning process built into the Incident Reporting Procedure (IRS) also helps to create an organizational culture that not only encourages incident reporting, but also the reporting of conditions and situations that could lead to a potentially serious accident, which we call high potential incidents (HPI).

We place great importance on and encourage reporting of HPIs with low real consequences, as they allow us to understand how our employees might have been harmed. We can then establish and implement control measures to rule out any risk of injury should a similar event occur again. These incidents are communicated throughout the organization, including senior management, to ensure accountability, impart knowledge and reduce the likelihood of a similar situation occurring again. By leveraging the reporting of HPIs, CRSs and holding monthly and quarterly meetings to review the findings of our investigations, we have improved knowledge sharing across the Company in relation to these situations but also about root causes and strategies to mitigate them. Our goal is to ensure that these problematic situations do not recur on any of our sites. These surveys are a key part of our leading indicators.
