Why Early Congenital Hypothyroid Screening is Important for Babies aged 48-72 Hours at Tebet Hospital, South Jakarta

Pediatrician at Tebet Hospital, South Jakarta, Dr. Ridha Kurnia Tejasari revealed that babies aged 48-72 hours must be included in the Congenital Hypothyroid Screening (SHK) to anticipate growth delays.

“The baby should be included in the SHK from the very beginning of birth, namely at the age of 48-72 hours,” said Dr. Ridha through a live broadcast held by the DKI Jakarta Health Agency (Dinkes), Tuesday.

This, according to him, is in accordance with the Regulation of the Minister of Health (Permenkes) Number 78 of 2014 concerning Congenital Hypothyroid Screening.

He said SHK was basically a thyroid hormone stimulation test to anticipate or detect Congenital Hypothyroidism (HK).

“SHK is for early detection of HK in order to prevent delays in growth/development of the baby and mental retardation (intellectual disorder/below average intellectual ability) in children in the future,” he said.

He explained, HK initially occurred due to deficiency (lack) of thyroid hormone. “This thyroid functions for the growth of nerve cells, the formation of brain tissue, the formation of bone structures in infants,” he said.

In essence, the thyroid hormone affects the growth of children in the future. “So it must be watched out for and anticipated from an early age,” he said.

If thyroid hormone deficiency is detected too late or a problem is indicated through screening, there is a risk that the baby will experience growth delays that lead to intellectual disability or mental retardation.

“This is the importance of SHK,” he emphasized.

SHK can only be done once and if the baby’s condition is stable. Stable means that the newborn is not in a sick condition, born prematurely, the baby’s weight, body length and head circumference are not normal.

“So the baby’s condition must be medically stable,” he said.

Regarding the initial SHK procedure, he said the mother and the baby’s family would be notified in advance before the baby was born.

“So before the mother gives birth or is pregnant, the mother or the family of the prospective baby is notified in advance that the prospective baby will be SHK,” he said.

If the baby is over 72 hours old, it can be examined within the first week of age. “If there are obstacles and if it is possible, then it is carried out at the age of under two weeks because the treatment is effective, it is attempted to be carried out at the age of the baby under two weeks,” he said.

If indeed SHK cannot be done because the baby’s condition is not yet stable, he said, then a re-examination will be carried out every month.

According to research, he said, if you find HK for more than three months and there are already clinical symptoms, then it is already a risk for the baby because medically it is too late.

“Indeed, in newborns there are no specific signs of HK symptoms,” he said.

If the baby’s condition is not yet possible to attend SHK within the specified age, the medical staff will still carry out SHK after the patient is stable and can be consulted.

He said that if SHK is done at normal times (age 48-72 hours), then the baby will be given therapy at the start. That is under two weeks after birth and the results will be very good in the baby.

“There will be no later generations with low Intelligence Quotient (IQ) or delays in all aspects of child development,” he said.

Apart from checking thyroid hormone levels, SHK is also intended to separate babies with abnormalities from normal babies.

For inspection procedures, SHK is carried out using the filter paper method.

“So two to three drops of the baby’s blood are taken from the lateral heel of the baby, then dripped on filter paper, dried and then sent to the laboratory. The results are at least two days or up to a week,” he said.

He said, HK cases generally occur in one in 3,000-4,000 births. “But as much as possible it should be known from the start so that it can be treated and the price of the medicine is also affordable,” he said.

He said SHK could be carried out at Advanced Referral Health Facilities (FKRTL) such as Puskesmas, RSUD, vertical hospitals and in general in laboratories.
