For persons who entered the Republic of Latvia before 11 October at 23.59, still had to fill out paper questionnaires, while from October 12 at 00.00 All persons entering the Republic of Latvia must fill in an electronic questionnaire on the website Covidpass.lv.
Completion of the certificate is mandatory for all immigrants and must be done not earlier than 48 hours before Border crossing of the Republic of Latvia.
What should be indicated in the confirmation questionnaire?
- Name and surname
- Identity number or travel document (passport or ID card) number
- Contact phone and email address
- The actual address where the person will be reached
- Countries where the person has stayed during the last 14 days before arrival in the Republic of Latvia
- Other supervisory authorities need information
How to fill in the family questionnaire?
- For each adult, the certificate must be completed separately, for minors, the certificate must be completed by parents or legal guardians.
After filling in the confirmation form, the immigrant will receive a screen and an e-mail personalized QR codewhich will confirm the submission of the data. The code is needed so that carriers, the Health Inspectorate, the State Police, the municipal police and border guards can make sure that the restrictions are met. The State Border Guard and the Center for Disease Prevention and Control will be able to access personal data as needed.
If the QR code is lost, the website will have the function “receive QR code” again by entering the e-mail address. If the questionnaire is filled in incorrectly, the certificate can be filled in again.
Personal data will be stored 30 days to use the information received to ensure epidemiological safety and traceability of contact persons. After 30 days, personal data will be automatically deleted.
Covidpass.lv must also be registered in case of transit through Latvia. When entering Latvia, transport and passenger transport service providers must also register each time.
Exception for certain border residents
On the other hand, Latvian border residents who study, work or receive medical services in Lithuania or Estonia, as well as residents declared in Valka municipality and Valga parish, who cross the Latvian land border with Estonia on a daily basis, an electronic confirmation questionnaire must be completed every 30 days.
At present, liability for non – completion of the declaration is provided by a fine of up to EUR 2000.
