Doctors are needed, especially if we want to reduce waiting lists. And for this reason in an order approved by the Government last summer directly to combat queues in health care, a strong discount was introduced – a flat fee of 15% – on overtime for white coats: in practice those doctors who work overtime are exempt from the additional services tax. It is a shame that immediately after the launch of the order, many Departments delayed to implement the measure by canceling it and paying in and starting, but a -now they have also decided that the tax discount will not be recognized for overtime work done at night, ie for those night shifts that they are most uncomfortable with. In short, a mockery that comes with a bitter taste of movement that for doctors gives only for minor increases, so much so that it is pushed to hit.
The flat fee is approved in June and paid in and starts
It all started last June when the decree on waiting lists was approved and then changed to law 107/2024 which included a tax incentive on services to encourage white coats to work more. A measure against a waiting list that worked immediately but remained suspended in several regions – according to a study carried out by the Cimo laboratory, for example Puglia, Emilia, Calabria, Trentino and Molise – which did not recognize immediate tax relief despite the law already in force. While in other Regions the situation was wild as in Sicily where the paradox was that in half of the hospitals the tax discount was applied and in the other half it was not applied. Local health authorities and the departments justified this situation by justifying themselves with the need for clarification on the implementation of the measure. And here is the second joke.
The Departments have decided to ban night shifts
In a document approved on November 7 last year, the ambitions of the Cimo-Fesmed doctors’ union, the Conference of the Provinces and Autonomous Regions decided to exclude additional services performed in night shifts from the fee favorable Movements in which, in addition – warns the union that produced this document – the discomfort experienced by doctors is much greater: “If this definition were applied, therefore, it would be difficult to find doctors willing to do the extra services at night”. In fact, Cimo Fesmed himself confirms how the doctors’ contract at this point is very clear: the “services watch night” are included – they explain – among the services involved in reducing waiting lists and, therefore, they must also benefit from the tax exemption at 15% on a provided by law 107/2024.
Doctors ready for appeals and the disappointing move
“We have written to the Conference of the Regions, the Ministry of Health and the Ministry of the Economy clarifying our position and therefore asking that the law be applied correctly – says Guido Quici, President of the Cimo Federation – Fesmed -. Otherwise, we are ready to flood the courts throughout Italy with an avalanche of appeals: we will ask members to report to us any erroneous claims of law.” Quici will also focus on his ‘ movement where it was promised, on the eve of the tax exemption from a special medical allowance, and “instead the draft budget law provides for an increase of only 17 euros per month. Now, even, there is an attempt to put our hands in the pockets of the doctors and even remove the few that have been recognized in the past months, asking them to pay back large sums for night shifts already done”, concluded Quici.
