Recently, the story of an Emergency Room (IGD) doctor who treated patients with unique and strange complaints went viral. A doctor named Rifki (30) once had a patient, a 16 year old teenager, who complained of bloody stools. The teenager admitted that he liked to eat rambutan and its seeds at the same time.
The story of his experience has been watched by 6 million Tiktok users. Many netizens are curious and surprised that a child could swallow such a big seed. To detikcom, Dr. Rifki told the beginning.
“Initially, this child came with complaints that his bowel movements had been bleeding for two days, the color was fresh red, there was a history of difficult bowel movements and pain when, for example, he wanted to defecate. He immediately said that he had eaten rambutan and its seeds 8 days ago,” said Dr. Rifki when contacted by detikcom, Tuesday (31/10/2023)..
Dr. Rifki said the incident occurred around the beginning of 2021, during the day. Previously, the teenager had also gone to another hospital for a check-up. However, at the previous hospital there was no surgeon, so a deeper examination could not be carried out.
“Actually, these rambutan seeds should be able to come out on their own through feces. It seems that they were actually being observed and were told to go home first, if for example there were complaints, then they were told to go to a hospital that had a surgeon. As far as I know, there was no surgeon at the previous hospital. During the observation period there was “There were still complaints at home and finally they were taken to this hospital,” explained Dr. Rifki.
“Then two days later, the symptoms appeared. He had gone to the hospital before, if I’m not mistaken, he was delivered by his uncle or a neighbor, in fact it wasn’t his parents who brought him. Two days ago he went to the hospital and 16 rambutan seeds were removed from his anus,” he continued.
Dr. Rifki then immediately checked with manual evacuation. The term is rectaltus (RT), namely inserting a finger into the anus to examine the anal muscles, mucosa or surface.
“We managed to manually evacuate it from the bottom, it was still there (the seeds) but it was a bit hard when you pried it open. It was difficult to get it out from the bottom. There were some that came out but they couldn’t evacuate all of them, so they were hospitalized. Well, when the manual was taken out the family was said, ‘Doctor, I think he’s still there because in his room there are a lot of snacks that he ate’,” said Dr. Rifki.
Apart from the bloody stools, the child complained of fever, nausea and pain in the anus. Initially, patients are given medication, such as laxatives, so that they can get out from under them during hospitalization. After that the patient was taken to the operating room for follow-up.
“In the operating room, evacuation is also carried out from below. However, in the operating room it is sterile, the equipment is complete, the PPE is better, like gloves, aprons, all kinds of things, and the room is also isolated, unlike in the emergency room. So, there is more freedom “carried out the procedure. The difference is that the patient was under anesthesia first, not like in the emergency room,” said Dr. Rifki.
“So, at the previous hospital it was 16, but in the ER here it was 35,” concluded Dr. Rifki.
