[Health Channel/Comprehensive Report]49-year-old Ms. Liao works in a hospital. When I was young, the first knuckle of my right index finger and middle finger would turn white, black, blue, red and swollen when exposed to cold weather. After blood tests, onychomycosis, peripheral vascular ultrasound, etc., the diagnosis was Raynaud Phenomenon. Yang Youlin, an endocrinologist at Miaoli Xin’an Clinic, shared that the clinical manifestations of Raynaud’s Phenomenon are clear-cut skin discoloration at the extremities of the limbs. The order is from white to blue, then to red. For treatment, the first priority is to avoid contact with cold environments. In severe cases, a doctor’s prescription may be required.

Yang Youlin’s Facebook page “Dr. Yang Youlin“The article explains that Raynaud’s phenomenon, also known as Raynaud’s syndrome, can be divided into two types according to the cause: primary and secondary:

Primary:Can’t find the reason. It usually occurs in young women aged 15-30 years old and usually has a family history. The only symptom was cyanosis of the peripheral limbs and no other complications. Smoking can worsen ischemic symptoms. The prognosis is better.

Recurrence:Combined with other diseases, common causes include the following:

●Autoimmune diseases, such as lupus erythematosus.
●Hematological diseases, such as cryoglobulinemia and chronic inflammation of liver C.
●Endocrine diseases, such as hypothyroidism.
●Vascular diseases, such as thrombosis, atherosclerosis, and vasculitis.
●Nervous diseases, such as carpal tunnel syndrome.
●Environmental factors, such as: vibration injury, emotional excitement.
●Drug-induced.

The cause is an overreaction of blood vessels. 3 symptoms to be suspicious of

Yang Youlin explained that Raynaud’s phenomenon is an obvious excessive vascular response to cold or emotional stress. The clinical manifestations are well-defined skin discoloration of the distal limbs, ranging from white to blue and then to red. When the extremities are exposed to cold air or ice water, the extremities turn white due to ischemia, followed by cyanosis, indicating tissue hypoxia. The ischemic phase usually lasts 15-20 minutes. As the temperature rises, vascular perfusion is restored, and redness and swelling of the skin ensue. There are 3 clinical symptoms for differential diagnosis:

●Are your fingers unusually sensitive to cold temperatures?
●Do your fingers change color when exposed to low temperatures?
●When the color changes, is the finger white or blue?

Fingers are most commonly affected and the first priority in treatment is to keep warm and prevent cold

Yang Youlin mentioned that Raynaud’s phenomenon most commonly affects the fingers, usually starting from one finger and then spreading symmetrically to other fingers. It occurs most commonly on the index, middle, and ring fingers. Other parts such as toes, earlobes, tip of nose, lips, nipples, etc.

Treatment methods: Keep warm and avoid contact with cold environments. In severe cases, a physician’s prescription may be required, such as calcium ion blockers, prostacycline, type 5 phosphodiesterase inhibitors, heparin or aspirin, etc. If the drug does not respond well, it is recommended to go to the plastic surgery department for sympathectomy.
