Understanding Diabetic Gastroparesis: Symptoms, Causes, and Treatment

[Voice of Hope, August 4, 2023](Editor: Li Wenhan) There are many chronic complications of diabetes. Among them, autonomic neuropathy in diabetic neuropathy can manifest as abnormalities in internal organs, such as nerves that direct the gastrointestinal tract to exit. The problem is manifested as diabetic gastroparesis. Normal people can move forward after eating, but patients with gastroparesis move backward, and often feel full in the upper abdomen, nausea, and vomiting. According to statistics, about 30% of patients with type 2 diabetes are accompanied by gastroparesis, and the number of patients continues to increase.

The pathogenesis of diabetic gastroparesis is related to many factors, among which the chronic damage of long-term hyperglycemia and gastrointestinal neuropathy are the main factors. This type of sugar lover often manifests as early satiety and postprandial bloating in the early stage, and may also have gastrointestinal symptoms such as nausea, vomiting, and abdominal pain. In severe cases, gastrointestinal absorption disorders may occur, and even completely unable to eat. This condition creates electrolyte imbalances, which in turn induce malnutrition. Moreover, the significantly prolonged time of food and drugs in the stomach will affect their absorption. Delayed drug absorption will lead to a decrease in drug efficacy, and delayed food absorption will cause a mismatch between postprandial blood sugar and the action time of injected insulin, increasing the risk of hypoglycemia. After the above-mentioned symptoms appear in sugar lovers, the possibility of gastroparesis should be considered, and the diagnosis should be further confirmed by gastric emptying test, except for organic diseases and mechanical obstruction.

The treatment of diabetic gastroparesis should mainly be carried out from the aspects of controlling gastrointestinal symptoms, nourishing nerves and stabilizing blood sugar. Gastrointestinal motility drugs are mainly used in the treatment of gastrointestinal symptoms such as early satiety and nausea. The occurrence of diabetic gastroparesis is closely related to poor blood sugar control, so the rational use of hypoglycemic drugs and control of blood sugar to reach the target is the key. Studies have found that insulin is more effective than other hypoglycemic drugs in gastroparesis patients with obvious gastrointestinal symptoms.

In terms of diet, on the premise of ensuring nutrition, patients with diabetic gastroparesis should try to eat less and more meals, and avoid smoking and alcohol. Usually, sugar lovers can appropriately reduce the proportion of cellulose foods in the diet, and avoid high-fat cooking methods such as frying, so as to accelerate gastric emptying.
