A breast biopsy is a procedure in which a doctor takes a portion of breast tissue from a suspected area, which is then examined in the laboratory by an anatomical pathologist to determine whether there are cancer cells. Biopsy is the main standard for diagnosing cancer, so a biopsy is very important to do.
A person needs a breast biopsy when symptoms such as lumps and tumors are found, or after breast radiology examinations such as mammograms and breast ultrasound are carried out, where the results of the examination show suspicion of breast cancer.

“Having a breast biopsy doesn’t always mean you have cancer. Most biopsy results from a tumor are not cancer, but a biopsy is the only sure way to find out whether there is malignancy (cancer) in the tissue taken. Breast biopsies are not dangerous and do not cause cancer. “The cancer spreads, like many myths spread,” explained Doctor Fransisca.
Broadly speaking, breast biopsies are divided into three, namely:
1. Surgical Biopsy with Open Incision
A biopsy is performed by making an incision in the skin of the breast and the doctor will remove the suspected tissue. This surgical biopsy is divided into two, namely incisional biopsy and excisional biopsy.
An incisional biopsy only takes part of the suspected area, while an excisional biopsy takes the entire tumor or suspected area, including a small amount of normal tissue around it. A surgical biopsy will cause sores or swelling and stitches will be required for post-procedure recovery.
2. Fine Needle Aspiration Biopsy (FNAB)
This biopsy is performed using a very thin, hollow needle attached to a syringe to withdraw a small amount of breast tissue or fluid from the suspected area. FNAB is most often performed in areas that are most likely fluid-filled sacs (breast cysts) and this method is classified as minimally invasive or minimally scarring surgery because the incision resulting from the biopsy is small.
FNAB may also be performed if the doctor is unsure whether the area seen on the radiology test is a small cyst (fluid) or a solid mass. However, because there is a small amount of solid tissue taken using the FNAB method, there is a possibility that the biopsy results will be less accurate.
3. Biopsy with a large needle/Core Needle Biopsy (CNB)
Compared with FNAB needles, CNB uses a hollow needle with a larger diameter to remove pieces of breast tissue in the suspected area. The needle is attached to a device that can move the needle in and out of tissue quickly, to extract more dense tissue cores.
This method is classified as minimally invasive surgery because the incision produced is smaller than a surgical biopsy. Just like FNAB, biopsy scars are usually very minimal but CNB has excellent accuracy because more sample tissue is taken.
Along with technological advances, CNB Biopsy has also experienced the development of methods or techniques, known as Vacuum Assisted Breast Biopsy (VABB). In the VABB procedure, the doctor will first localize the tumor area using a radiological tool, such as a breast ultrasound, then take a sample using a sophisticated vacuum tool in the localized area.
Doctor Francisca also revealed that VABB technology has several advantages for patients because the biopsy incision is very minimal, the risk of infection is minimal, the duration of the procedure is faster, the accuracy is better, and the recovery after the procedure is faster.
Oncology Surgical Specialist from Mayapada Hospital, South Jakarta, who has experience performing many VABBs, Doctor Bayu Brahma, explained that this technique is very useful for cases that do not necessarily require surgical biopsy, for example in cases of tumors that are suspected to be benign.
“In the case of benign tumors with certain conditions, for example very small ones, VABB can even be done at the same time to remove the entire tumor, so that the patient does not need open surgery or a more invasive surgical process,” he explained.
Apart from using sophisticated tools, performing VABB also requires the hand skills of an oncology surgeon who has undergone special training, so not all health care facilities can carry out this procedure.
Recently, DDoctor Francisca Badudu successfully performed a VABB procedure on a patient who complained of a lump in the breast that required a biopsy. Finally, the patient underwent a VABB procedure and received treatment at Mayapada Hospital Bandung.
This procedure is also the first VABB procedure in the city of Bandung because currently only Mayapada Hospital Bandung can perform breast biopsies using the VABB method in that city.
