What is the underlying cause?
Before hemorrhoids and other anal complaints can be treated, the underlying cause of the problem must first be clarified.
The research into this is done in the Proctological Center. Proctology is the field that deals with physical conditions surrounding the anal area.
First date
A referral to the Proctology Center always goes through your GP or medical specialist. Your appointment will be scheduled with the proctologist-dermatologist or the proctologist-surgeon. Our center also employs trainee physician specialists, with whom you may sometimes come into contact. They always work under the responsibility (supervision) of the dermatologist or surgeon. If you prefer a specific medical specialist, you can indicate this to your GP. He will mention this upon referral.
At the scheduled appointment, the doctor will first discuss your complaints with you. To determine the cause, an anal examination is usually carried out in advance. This examination takes place during the first outpatient clinic visit using an anal telescope (proctoscope). The examination is carried out by the proctologist-dermatologist or proctologist-surgeon. During your first visit, he will explain his findings to you and make a proposal for treatment.
Research
During your first visit, the doctor will perform proctological examination using a proctoscope. The examination is called proctoscopy. During this examination you will lie in a gynecological chair. The examination takes approximately 10 minutes.
Need additional research?
It may be that the proctological examination has not provided sufficient information and that additional (intestinal) examination is necessary. Your doctor will inform you about this.
Therapy
After the proctological examination, the doctor will explain his findings to you. He will then make you a proposal for treatment. In some cases, if you agree, treatment can take place immediately after the proctological examination. It is therefore useful to take someone with you who can take you home after the treatment.

The examination and treatment of anal disorders consists of several steps. We call this treatment process a care path. In the ‘Anal disorders’ care pathway, you may encounter various practitioners who are involved in your treatment.
Condition
What is it and which treatment is suitable?
Hemorrhoids
Swelling cells on the inside of the sphincter muscle, which sag due to too much pressure.
The treatment may consist of:
lifestyle advice and diet; treatment with rubber band ligatures (Barron ligature). The sagging mucous membrane is tied off with elastic bands and dies; injections of sclerosing (shriveling) fluid. The liquid is injected under the hemorrhoid to ‘hang’ the mucous membrane back into its original position; laser treatment according to LHP (Laser Hemorrhoidal Plastics) method; surgical treatment (rarely necessary).
Fissures
Cracks in the skin or mucous membrane of the anal skin.
The treatment may consist of:
cream that relaxes severely cramped muscles. This should be used for 6 to 12 weeks; botox. This remedy temporarily relaxes the cramped muscle, so that the mucosal tear has time to repair; small, targeted cut in the mucous membrane and part of the muscle.
Eczema due to contact allergy
Contact allergy is an allergy to substances that come into contact with the skin. The treatment consists of detecting the responsible substance(s) by means of allergy tests and targeted treatment of the eczema.
Seafood
Skin flaps around the anus, usually very harmless. Treatment is not necessary.
Fistulas
Corridors in and under the skin. First, the depth and extent of the fistula formation will be determined using X-ray examination. Depending on this, laser or surgical treatment will take place.
Itch
This can be caused by hemorrhoids, fissures, eczema, marissas, fistulas, infection and so on. The proctological examination can provide more information about this. Depending on the cause, treatment is started. The use of an ointment serves to support this.
Swelling
These can be caused by, for example:
genital warts (warts located on or around the genitals) or external hemorrhoids, prolapse of the mucous membrane, but also due to benign or sometimes malignant conditions; In the elderly, weakened pelvic floor muscles can cause this complaint. Targeted pelvic floor training through a specialized physiotherapist can then be a good option; The complaints may also be related to intestinal diseases such as constipation, Crohn’s disease, ulcerative colitis or other intestinal infections. In that case, we will conduct research together with the gastrointestinal and liver doctors.
