The Thuismeten App: Support and Care After a Stroke From the Comfort of Your Home

The treatments for a stroke (CVA or TIA) have improved enormously in recent years. The hospital stay is often short. Patients recover better and feel better faster. This means that they can also go home earlier. But once home, there are often still questions or a patient experiences certain limitations.

The Home Measurement app is an app that supports you during the first 12 weeks after your discharge from hospital. With this app your practitioner can guide you remotely. You do not need to go to the hospital for this. You can simply keep in touch with your healthcare provider from home or elsewhere.

In the Home Measurement app you will find information that can help you with questions that arise at home. But you can also share data with us so that we can help you remotely. So you can share information with your healthcare provider and vice versa in a simple and secure way.

Why care close by?

With the Thuismeten app you send us data about your health. We can then provide you with targeted information or assistance. This will give you more insight into your health. This helps with your daily functioning and improves your quality of life and well-being.

In the app you will find information in the form of text and videos. The information is about lifestyle or what you can do to reduce (physical) complaints. But you can also find advice on dealing with invisible consequences. Such as fatigue, emotional or behavioral changes and overstimulation.

The Thuismeten app therefore helps to offer care that suits your personal wishes and needs as much as possible. We believe it is important that you can decide for yourself which care suits you best.

In the first 24 weeks after your stroke you can use the Thuismeten app

  • The nurse from the Medical Coordination Office will guide you in the first 12 weeks.
  • After this period, we will ask you via the app whether you would like an appointment at the outpatient clinic. This could be a telephone appointment, video calling appointment or an appointment at the outpatient clinic.
  • Would you like an appointment earlier? Then indicate it via the app.
  • Once the first 12 weeks are over, you can no longer ask questions via the app. You can then use the self-care modules on the various topics for another 12 weeks.

Your care provider will register you for care nearby. To use the Thuismeten app, you need a smartphone or tablet. You must download the Home Measurement app from the company Luscii on your smartphone or tablet. Below is a step-by-step explanation of how to do this.

  • You will receive an email from Luscii about the Thuismeten app at the email address you provided. You can then install the Thuismeten app. It is important that you can read this email in the hospital so that we can explain the Home Measurement app to you in the hospital.
  • If you do not see the email immediately, check the spam or unwanted email.
  • You download the Thuismeten app on your phone or tablet via the App Store or via the Play Store.
  • To complete the registration, you will receive a verification code by email. Enter this code within 15 minutes, otherwise the code will expire and you will have to register again.

Today
Here you will find the measurements you need to take that day. For example, these are the questions you need to complete.

Behind each action is a blue button.
Have you already submitted a promotion? The home button changes from blue to gray.

You will receive an email from us and a notification in the app if you need to take a measurement that day.
Do you find the email unnecessary? You can change this in your settings.

It is always possible to take an additional measurement. To do this, click on the gray button next to the measured value. This way you can take an additional measurement.

Self-care

This section (see Figure 1) contains information about how to deal with the consequences of a stroke. You find:

  • information about the signs of a stroke;
  • information about stroke treatment;
  • tips for dealing with the consequences of a stroke;
  • tips for a healthy(er) lifestyle;
  • tips for dealing with fatigue.

To report

Sometimes your healthcare providers want to contact you. For additional questions, for example, or an extra questionnaire. They often contact us via video calling or by telephone. But they can also send you a message. You will then see that message under ‘messages’.

If you have a question for us, you can also send it to us via ‘messages’. Your healthcare provider will then call you back.

Measurements

We regularly ask you to complete a few short questions via the app. For example, about how your fatigue is going. Or how it goes with learning to deal with the consequences of a stroke.
We will use your answers to help you further. For example, sometimes you get a tip to read something in the app.

Your healthcare providers at the hospital can see your measurements as soon as you send them. If they notice that you are doing less well, they will call you.

PROMS

We believe it is important that you receive good care. Good care is about your quality of life. That is why we ask you to complete so-called PROMS in addition to questionnaires. PROMS stands for Patient Reported Outcome Measures. These are questionnaires in which we ask you how you feel and how you are doing. The questions are, for example, about fatigue, self-reliance and your physical and mental recovery after a stroke.

You will receive a message in the app to complete a PROMS questionnaire. This message contains a link to MijnIsala.nl. We have prepared the questionnaires for you here.

Are you having problems with your own device (iPad, tablet, smartphone) and you are unable to complete a measurement? Please let us know that you cannot complete a questionnaire or measurements.
