The mRNA vaccine also works in people with a congenital immune disorder

Vaccinations usually do not work or are less effective in this patient group

The majority of people with a congenital immune disorder get an immune response after an mRNA vaccination against corona. This is apparent from research conducted by Erasmus MC. Good news, because usually vaccinations do not work or are less effective in this group of patients.

Clinical immunologist Virgil Dalm of Erasmus MC and colleagues investigated whether the corona vaccine works in patients with a congenital immune disorder. An immune response was found to be initiated in 80 percent of this patient group. The participants made antibodies and had a T-cell response. In total, 500 people were vaccinated twice with the Moderna vaccine. The results have been published in the scientific journal the Journal of Allergy and Clinical Immunology.

The results are important, because Dalm’s patients have had a difficult time. If you have a congenital immune disorder, such as cvid, you get more quickly and more seriously ill from infections. In addition, vaccines often fail. ‘I know families who have been indoors for the full two years, for fear of getting sick and ending up in the ICU. And while the rest of the Netherlands waited for the vaccine, my patients wondered if there would ever be a solution for the corona virus for them.’

Dalm is not yet sure why the mRNA vaccination works better than other vaccines in this patient group: ‘These vaccines may stimulate the immune system more strongly than other vaccinations.’

Good news in any case for people with a congenital immune disorder. ‘That’s for sure’, confirms Dalm, but especially now that the omikron infections are still very high and there are many flu infections, he advises his patients to pay attention. Because although the vaccine seems to trigger an immune response, patients with a congenital immune disorder can still get quite ill.

‘I advise my patients not to take the metro to the Koopgoot. A dinner with friends where everyone does a self-test is possible. As a rule of thumb for my patients, I stick to: try to avoid situations where you have no control for a while.’

Collaboration
The patient association was involved in the research, which was subsidized by ZonMw SAS and all university medical centers in the Netherlands. Dalm: ‘It’s an awful great collaboration, which was set up in a very short time due to its great importance. Suddenly all noses were in the same direction and we could all do this research together.’

Within Erasmus MC, the departments of Internal Medicine, Clinical Immunology and Viroscience were involved, including Rory de Vries, Corine Geurts van Kessel and Leanne van Leeuwen. Virgil Dalm was in charge of the consortium.
