LThe health system is facing an “unprecedented crisis”, particularly in the public hospital. Faced with this observation, on Monday 7 November the National Ethics Advisory Committee (CCNE) published an opinion calling for a rethinking of the health system “on an ethical basis” that “would put the person at the center of his concerns”. The CCNE presented this opinion last week to the Minister of Health, François Braun, one month after the launch of the health component of the National Council for Refoundation (CNR).

Focused on the tense situation of the public hospital, “the most salient symptom of the crisis”, and taking note of the “weaknesses” revealed by the Covid-19 pandemic, the text denounces a discrepancy between the practices of caregivers “perceived as dehumanized and” ethical values ​​”, which are explained in particular by an approach to health focused on“ treatment rather than cure ”.

“The patient’s word is not sufficiently taken into consideration, as well as the moments of confrontation between caregivers. It is not valued because it is human time that is not subject to pricing ”, explained to the Agence France-Presse Régis Aubry, rapporteur and head of palliative care at the Besançon University Hospital. However, combined with “managerial practices” and “increasing technicization”, this “systemic” approach has led to “a deterioration of the care relationship in a system where caregivers’ time increasingly crosses that of patients”, according to CCNE.

A reform of the health system

“This is one of the keys to understanding the current desertion of caregivers, added Régis Aubry. They no longer understand why they heal and are in ethical suffering ”. All the more so since salaries remain “low compared to the required commitment”, notes the CCNE.

Faced with this observation, the speakers underline that ethics is “neither optional nor optional” in health policies, and ask for a reform of the health system “that puts the person at the center of his concerns”. Two principles are essential in this reform: “Equal access for all to the health and welfare system and unconditional respect for the people treated and those who care for them”, according to the CCNE. “Since the minister believed that what we have written should serve as a compass, we will be vigilant on the proposals of the CNR health”, warned Régis Aubry.
