The Critical State of Occupational Medicine in the Pithiviers-Malesherbes Sector

In a letter sent in June 2023 to companies in the north of the department who are members of the Loiret inter-company hygiene committee (CIHL), its director Bruno Antoinet warns of the current situation of occupational medicine in the Pithiviers-Malesherbes sector.

“There is a lack of occupational doctors everywhere and the situation is getting worse and worse. All this to the detriment of companies and employees in the territory for their health monitoring”, he summarizes. Currently, there are only two part-time occupational physicians (one three days a week, the other two and a half days) to provide follow-up consultations for 11,000 Pithiverais employees. Not to mention temporary workers.

“We are no longer within the regulatory deadlines”

Trained nursing staff support the doctors. There are two nurses and a third from another sector, who comes as reinforcements. The consultations take place in a building in the Senives area in Pithiviers, premises in Malesherbes and the doctors and nurses may have to travel to certain large companies in the area.

Regarding the two doctors, “they are in cumulative retirement employment. They are retired doctors who continue to practice so it will not last forever”, advances Bruno Antoinet.

Current result: “We cannot function properly. There is a delay that we cannot reduce. We cannot meet our obligations knowing that we do not have enough medical time. is more within the regulatory deadlines compared to the needs”, deplores Bruno Antoinet. If the situation has been critical for several years, “medical time has never been as low as it is today.”

“We have no job prospects”

And the future seems blocked. “We have no job prospects,” adds the director of the CIHL. “When we offer trained caregivers to come here, they don’t want to. The problem is also more global today because we train few occupational doctors.” General practitioners who would like to reorient themselves towards occupational medicine must redo four years of study. As for foreign doctors, outside the European Union, they too must pass a competition (knowing that they must be French-speaking and already be in the field of occupational medicine) if they want to practice in France.

Tensions in the labor market are also having an impact. “There is turnover, which means that we are seeing an increase in the number of hiring visits”, specifies Bruno Antoinet. And to add: “The health problem linked to the absence of attending physicians affects occupational medicine.”

Priority to so-called urgent visits

Currently, because of this situation, priority is given to so-called urgent visits, “for recoveries, pre-recoveries following sick leave.” For others covered by classic monitoring, the deadlines are getting longer. “It’s really not satisfactory,” admits the director of the CIHL. “Companies are wondering: why do they pay a contribution if we cannot meet their demands? “

The Regional Directorate for the Economy, Employment, Labor and Solidarity has long been informed of this situation. “We have been alerting for years. All we are told each time is that there is a problem of medical shortage.”
