Temporary Interruption of Endocrine Therapy for Pregnancy Wish in Young Breast Cancer Survivors: No Increased Risk of Short-Term Breast Cancer Events

In selected women with a history of early hormone receptor-positive breast cancer, temporary interruption of endocrine therapy due to the desire to conceive did not increase the risk of short-term breast cancer events, compared to an external control cohort. Further follow-up is needed to investigate longer-term safety.

The POSITIVE study assessed a temporary interruption of adjuvant endocrine therapy due to a pregnancy wish in young women with a history of breast cancer. They were eligible to participate if they were 42 years or younger; had stage I, II, or III disease; received adjuvant endocrine therapy for 18 to 30 months; and had a pregnancy wish. The primary endpoint was the number of breast cancer events (defined as ipsilateral or locoregional invasive disease, distant recurrence, or contralateral invasive breast cancer) during follow-up. The outcomes in this interruption group were compared to those in an external control cohort consisting of women who would have met the inclusion criteria for this study.

The 516 participants had a median age of 37 years, the median time from breast cancer diagnosis to inclusion was 29 months, and 93.4% had stage I or II disease. Of the 497 women followed for pregnancy status, 368 (74.0%) had at least 1 pregnancy and 317 (63.8%) had at least 1 live birth. A total of 365 babies were born. After 1,638 patient-years of follow-up (median follow-up: 41 months), 44 patients had a breast cancer event, which did not exceed the safety threshold (46 events). The 3-year incidence of breast cancer events was 8.9% (95% CI 6.3-11.6) in the interruption group and 9.2% (95% CI 7.6-10.8) in the control cohort.
