Currently, there are three infectious diseases that are very likely to be transmitted from mother to fetus, namely HIV infection (Human immunodeficiency virus), syphilis and hepatitis B. These three infections and diseases can cause morbidity, disability and death for mothers and can reduce the quality of life of affected children. One of the steps to overcome this problem, the government implements a program triple elimination to control this disease through the Regulation of the Minister of Health of the Republic of Indonesia Number 52 of 2017 concerning the elimination of the transmission of HIV, Syphilis and Hepatitis B from mother to child.

More than 90% of cases of HIV infection, syphilis and hepatitis B found in children occur due to transmission from the mother. Data from the Report of the Ministry of Health of the Republic of Indonesia in 2018 shows that the prevalence of HIV-infected pregnant women is 0.39%, syphilis infection is 1.7% and infection from hepatitis B is 2.5%. In 2020 there were 6,094 HIV-positive pregnant women and 4,198 positive pregnant women for syphilis in Indonesia, while Dr. Sardjito Yogyakarta himself in 2020 found 4 pregnant women with syphilis, 30 pregnant women with HIV and 18 pregnant women with hepatitis B This should be of great concern to the public so that they are more aware of the dangers of this infection, both for the mother and for the mother for her child. To break the chain of transmission of these three infectious diseases, we must do it together because the patterns of transmission of these diseases are relatively the same, i.e. from sexual intercourse, blood exchange / contamination and vertically from mother to child, both during l uterus, during the delivery process. , as well as during breastfeeding. The transmission of this infection from mother to child can effectively be prevented with simple and effective interventions, particularly in the form of early diagnosis or screening during early pregnancy, treatment as early as possible when diagnosed, and immunization. Lack of knowledge and information on HIV infection, syphilis and hepatitis B during pregnancy checks resulted in a high transmission of these infections from mother to child. Kedaung Barat Public Health Center is a rural health center located in the East Sepatan district, has 105,578 residents with a number of pregnancies around ± 1900 each year. Health checks during pregnancy are mandatory for pregnant women to ensure that the mother and fetus are in good health during pregnancy, as well as to avoid complications during pregnancy and even during childbirth which risk causing emergencies and even death for both mother and fetus. At least pregnant women check their content 6 times during pregnancy. The pregnancy exam consists of a physical exam and a laboratory. Laboratory health examination is an effort to diagnose the disease early. Pregnant women need to undergo a laboratory examination once during pregnancy. Health checks for pregnant women can be done in Puskesmas, Posyandu or at the Independent Practice Midwives (BPM) in the East Sepatan district. There are 14 BPMs in the East Sepatan district. For laboratory tests, there are only Kedaung Barat health centers, as there are no privately owned laboratories in the East Sepatan district. Non-coverage of laboratory tests for pregnant women in 2020 (equal to Hiv: 1901 (84.45%) Syphilis: 1938 (86.09%), Hep B: 1896 (84.22%)) becomes the basis to realize this innovation, because if left untreated, it will increase the risk of complications experienced by pregnant women but detected early, so that it can cause emergencies and even death of pregnant women and their fetuses. The laboratory health examination is part of the pregnancy health examination according to the minimum service standards (SPM) for Puskesmas as established by the Regulation of the Minister of Health n. 4 of 2019 on the Minimum Service Standards for Puskesmas.

With the existence of laboratory tests for pregnant women, it can be used as an initial screening / early detection of a disease which can then be done with proper management from an early age if a disease is detected from the test results . a disease that can therefore be performed early with proper management if a disease is detected from the examination results

The SI IBU PERI (Alert for Pregnant Women for Laboratory Examination) innovation aims to ensure that all pregnant women in the West Kedaung Health Center work area receive laboratory tests so that they can break the chain of health problems.

The SI IBU PERI innovation target is all pregnant women with a gestational age of 0-12 weeks. Laboratory tests were then performed, including Hb, blood group, glycemia, syphilis, HIV and HBsAg, and urinalysis consisting of protein and urinary pH.

Public service innovation is a breakthrough in the type of public service, whether it is an original creative idea / idea and / or adaptation / modification that benefits the community, directly or indirectly. Public service innovations in the health sector must be continuously developed in order to improve the health of the community. The SI IBU PERI (Alert for Pregnant Women for Laboratory Examination) innovation aims to ensure that all pregnant women in the work area of ​​Kedaung Barat Health Center receive laboratory tests so that they can disrupt the chain of HIV transmission, syphilis and hepatitis B through the elimination of transmission. The continuation of this innovation needs to be evaluated on a regular basis in order to provide better public services and overcome health problems. In addition to paying attention to the needs of the community so that other innovations can be planned that can break the chain of other diseases.
