The caregivers of Lagrave and Cadalen held their monthly meeting in the premises of the nursing home. The purpose of these multidisciplinary consultation meetings (RCP in the jargon) is to take a cross-look at complex patient cases in order to improve care. They are established within the framework of the functioning of the nursing home. This deserves some explanations that Dr. Joëlle Favarel was kind enough to give us.

What is meant by a nursing home?

At first glance, of course, this is an establishment where health professionals are gathered. But, upstream, it is a health project established following a diagnosis carried out by the caregivers in a given territory. This project is validated by the Regional Health Agency. Health insurance agrees and controls it. It is managed by a SISA, Interprofessional Society for Ambulatory Care. A coordinator accompanies him. 15 caregivers, doctors, nurses, dentist, physiotherapists, podiatrist, speech therapist, pharmacists joined this operation and cover the living areas of Lagrave and Cadalen.

What are the axes of the Lagrave-Cadalen health project?

Each practitioner works in the service of his own patients. But together we implement public health missions with a multi-professional approach. For example, improving immunization coverage, identifying and monitoring elderly people at risk of frailty or even monitoring chronic pathologies, child development disorders. Everyone can train and gain skills on these subjects. We have pooled the IT tool essential for sharing data.

This operation has already existed for 2 years. What lessons can be learned from this?

First of all, better organization: we have set up on-call duty on Saturday morning and evening until 8 p.m. Then, pooling our skills is an undeniable asset. A team dynamic was created. We learn from each other. Moreover, despite a difficult context, the healthcare offer is evolving. A dentist and a physiotherapist have joined the team and we hope for the arrival of a new doctor.
