Huang Junde, the attending physician at the Department of Intensive Care Medicine at Taichung Wing General Hospital, said that before the Chinese New Year, my nephew ate sashimi from conveyor belt sushi and suffered from severe septic shock combined with acute liver and kidney failure. The mortality rate from liver failure was as high as 70%, and he had to undergo liver and kidney dialysis. It was only later that he was rescued, “The past seven days have been extremely stressful.”
Huang Junde posted on Facebook that on the 3rd, he went for a run and received a call from his brother. “My nephew is very unwell. The doctor at the clinic recommended that he be transferred to the hospital. Since there is no outpatient clinic on Saturday, I asked my brother to take him directly to the emergency room and end the original plan early.” After a long-distance run, I went home to take a shower and prepare to go back to the hospital. My nephew was discharged from another medical center after ERCP due to common bile duct stone obstruction last month. I was thinking, could it be recurrence of bile duct inflammation?”
Huang Junde said that he checked on the computer and found that his nephew’s “blood pressure was only 50/27, heart rate was 130, and respiration was 35! I immediately canceled all weekend trips and rushed back to the emergency room! The emergency room was full that day, and I walked through the busy emergency room. Finally, I found my nephew’s bed next to the door at the bottom of the corridor. The first-line vasopressor was full. After asking, I was given 2500ml of infusion. My blood pressure was only in the early 70s. I was groggy and breathing shallowly and quickly. I quickly called for help. After taking the second line of vasopressor and Solu-Cortef pump, the blood pressure slowly rose to 90, and the mean arterial pressure was 50.”
Huang Junde pointed out that after performing an abdominal computed tomography and drawing blood on his nephew, it was found that he was suffering from severe septic shock combined with acute liver and kidney failure. “I quickly contacted MARS+CVVHDF to start liver and kidney dialysis, and contacted the chief physician on duty for coordination. I was admitted to the intensive care unit. At this time, the computed tomography scan came out, and surprisingly, no evidence of bile duct obstruction was found.”
Huang Junde said, “After confirming that there was no immediate need to place any drainage tubes, I quickly contacted the hematology and oncology department to rule out the possibility of malignant tumors. I also asked the chief physician of the nephrology department to place the dialysis catheter, and then went to the first intensive care unit to follow up in person. The resident doctor on duty and the leader on the night shift take over the shift and explain the next treatment plan.”
Huang Junde pointed out, “After six days of hard work, my nephew was finally brought back from hell. He was successfully transferred to the general ward last night. When I heard him complaining that the porridge in the hospital was unpalatable, I knew that this brat should be back.
