So, for women whose marriage has already been dissolved, it is not always easy to confirm their maiden name. This is because a birth certificate is not suitable here, and other documents cannot be preserved.

It is noted that it is necessary to obtain an extract from the state register of civil status acts. This can be done in any department of the registry office, regardless of the place of state registration of the civil status act and the place of residence of the applicant.

What documents need to be submitted

You also need to submit an application / request, which indicates information about the person in respect of whom an extract from the register is issued:

• Full name, date and place of birth of the person regarding information about whom the request was received;
• the series and number of the certificate of state registration of the corresponding civil status act, the date of its issue;
• registration number by which the entry of information about the act of civil status is registered.

In addition, in the case of filing an application or request for an extract from the register regarding the deed of marriage or divorce, the surname, name, patronymic of the second spouse is additionally indicated.

Therefore, the applicant can apply for such a document in person, namely:

• directly to the department of state registration of acts of civil status;
• by sending documents by mail (by registered mail).

An individual, at the time of personal filing of an application / request for an extract from the Register, must present a passport of a citizen of Ukraine or a passport document and, if appropriate, documents confirming kinship or family relations.

The authenticity of the applicant’s signature on the application, as well as the correctness of copies of the passport of a citizen of Ukraine or passport document and documents confirming kinship or family relations, must be properly certified.

The fee for issuing an extract from the register is 73 hryvnia.
