Obesity (BMI ≥30) is a risk factor for venous thromboembolism in pregnancy and puerperium. This risk is higher as the BMI is higher. However, the guideline working group could not find any supporting evidence in the literature for the routine prescription of thromboprophylaxis after vaginal delivery.
For a caesarean section, a prolonged or repeated dose of an antibiotic can be considered as perioperative prophylaxis in obese women. The benefits and risks must be weighed against each other. It is indicated that the tissue concentration of an antibiotic is partly influenced by body weight, and that obese women have a higher risk of infections. Therefore, it seems logical to extend the dose in obese pregnant women. But the risk of side effects is higher and it is still unknown what the risks of the extended treatment are on the child’s microbiome.
