The Nipissing Ontario Health Wellness Team (NHWT) is a new model of health care coordination and delivery that aims to provide patients and caregivers in the Nipissing area with better access to the services they need. Its primary goal is to support patient outcomes by consolidating services and facilitating access to the local health care system.
Our Ontario Health Team (OHT) has over 30 members with expertise in a variety of areas including home and community care, social and support services, digital health, mental health and addictions, primary care, long-term care and seniors’ residences, acute care, children’s services, Indigenous services and social services.
It is essential that patients and families be involved in the development of the ESO and its decision-making process. The Patient, Family and Caregiver Council (PFCC), formerly the Ontario Health Team Patient and Family Advisory Council, will play a critical role in improving patient experiences and health outcomes by providing patients and their families with the opportunity and means to contribute to reimagining health care delivery in the region.
The Nipissing Ontario Health Wellness Team (NHWT) is funded by the Government of Ontario.
