Justifications to produce as to the qualities and capacities of the candidate:
Documents to be produced by the candidate, in support of his candidacy:
– Copy of the judgment (s) pronounced, if the candidate is in receivership;
– Declaration on the candidate’s honor justifying that he does not enter into any of the cases of mandatory prohibition to tender provided for in articles 45 and 48 of ordinance no 2015-899 of 23 July 2015 or, for public defense contracts or security, that it does not enter into any of the cases of mandatory prohibition to tender provided for in articles 45, 46 and 48 of ordinance no2015-899 of 23 July 2015 (if this declaration is not already requested in part of form DC1, below);
– Declaration on the candidate’s honor attesting that he is in order, during the year preceding that during which the launch of the consultation takes place, with regard to articles L. 5212-1, L. 5212- 2, L. 5212 5 and L. 5212-9 of the labor code, concerning the employment of disabled workers.
Documents to be produced in support of the candidacies by the candidate, at the choice of the public purchaser:
– Declaration concerning the overall turnover and the turnover concerning the supplies, services or works covered by the contract, carried out during the last three financial years available;
– Appropriate declaration from banks or proof of insurance for professional risks;
– Statement indicating the candidate’s average annual workforce and the number of supervisory staff for each of the last three years;
– Presentation of a list of works carried out over the past five years, supported by certificates of good execution for the most important works. These certificates indicate the amount, time and place of performance of the work and specify whether they were carried out according to the rules of the art and regularly completed;
– Declaration indicating the tools, material and technical equipment available to the candidate for carrying out contracts of the same nature.
