Integrated Care Agreement Unites 50+ Organizations to Combat Healthcare Challenges in the Midden-IJssel Region

A year after the signing of the national Integrated Care Agreement and after extensive research in the region, more than 50 organizations have joined forces with residents. Salland Zorgverzekeraar, municipalities of Deventer, Olst-Wijhe, Raalte and Voorst, residents, Salland United, which within the Salland United network includes general practitioners, nursing and care homes, home care, care for the disabled, mental health institutions, hospitals, welfare organizations, have united to draw up the regional plan.

Challenges

Earlier this year, research was conducted into the population’s use of care and the capacity and performance of care, welfare and support. For example, it appears that approximately half of young adults experience mental complaints. The region is aging faster than the national average. The number of residents with excessive alcohol consumption and obesity is higher and rising faster than in many other regions in the Netherlands. Health differences, even at district level, are large. And unfortunately, the shortage of healthcare personnel is also increasing in Midden-IJssel. All this means that the demand for care is increasing, while there are fewer people to provide this care.

Ambition

The regional plan focuses on addressing the most pressing challenges facing healthcare in the region. The joint ambition is that by 2033 Midden-IJssel will be a region where there are equal opportunities for the healthiest possible life for everyone, where it is pleasant to live, even if you need care or support, where professionals can do their work comfortably, where healthcare is smartly organized and we have resilient and supported communities.

Approach

The regional plan contains concrete agreements on how the most urgent challenges will be tackled by the collaborating organizations to achieve these goals:

Healthy living

Residents enjoy living in the region and experience good, accessible care nearby. More residents in neighborhoods with a low socio-economic status experience better health through a healthy lifestyle and making healthy choices. Collaborating organizations strive for a strong community; residents are more available to each other and experience strong solidarity in the environment. In addition, the regional plan focuses on positive health. And that means not only focusing on diagnosis, treatment and prevention of diseases and conditions, but also on a person’s well-being and resilience; what someone can do.

Care and support in an aging society

Elderly and disabled people live independently at home for longer and experience good accessible care and support nearby. The supply of suitable homes with professional care and support and commitment from society to live independently at home for longer has increased.

Professional central

Healthcare workers enjoy working in the region. Due to a lower workload and less administration, they experience more time for the patient, more job satisfaction and resilience. Outflow of healthcare personnel to a profession outside the healthcare sector, the level of absenteeism and staff shortages are decreasing.

Smarter organization of care

Care is organized differently with more use of technology, smarter collaboration and seeing what the resident can do themselves. Residents who need care or support experience shorter waiting times and good quality of care and support.

Mental well-being

There is less loneliness in the region and fewer people experience mental problems. More citizens are more likely to be there for each other and support each other where necessary; there is a strong sense of solidarity in society. The demand for mental health care (GGZ) has decreased and there is better coordination of the existing care demand, which also means that waiting times for mental health care are shorter.

What now

To achieve the goals, the collaborating organizations have established work packages and monitoring of developments and results. The work agenda for the coming years is in the making and all organizations and residents, clients and patients are involved to make developments in the region and this healthcare transformation a success.

Future vision

With the plan, organizations from the healthcare, welfare and social domains are committed to a future in which good care, well-being, support and a strong sense of community go hand in hand. The plan and the region have everything to make healthy living easier and keep good care accessible and affordable for everyone in the region.
