Events: Scheduling a third dose of covid vaccination

Philharmonic Roman Novotný received new lungs less than four years ago. He goes to the Motol hospital for regular check-ups. And right in the winter, he was also one of the first to be vaccinated here. “If the covid took my new lungs, it would probably be the end,” he said.

However, due to medication, vaccination is not as effective in transplant patients. A strengthening third dose would therefore be welcomed. “Protecting your new lungs and functioning as before is an absolute priority. So I will definitely go to the third dose, to revaccination, “Novotný advises.

“After two doses, we performed a very careful examination and found that they had almost no antibodies, or actually no antibodies, and very low cellular immunity. The next dose, or in combination with another vaccine, is something very topical, “says Head III. Department of Surgery, First Faculty of Medicine, Charles University and Motol University Hospital Robert Lischke.

According to the Minister of Health, this group could get it already in the autumn. Other at risk could be the elderly or the chronically ill. However, the decision on a booster vaccination will not be made until the results of clinical studies have shown how long immunity in humans will last after injection.

Vaccines are, vaccination centers will not

“These vaccines are already being produced and clinically evaluated and have a somewhat modified composition to reflect the delta, delta plus variants,” describes the chairman of the Czech Vaccinological Society, Roman Chlíbek.

“We have to learn to live with the virus and vaccination is one of the ways,” says Zdeněk Hostomský, director of the Institute of Organic Chemistry and Biochemistry of the Academy of Sciences.

“I would take the third dose if a variant appears that is different and resistant to normal vaccination. Then I will take the third dose, which will be adapted to the new hypothetical variant, but I would not take the third dose of the same, “adds Hostomský.

“We have vaccine orders set up so that we are able to revaccinate the entire population if needed,” reassures Health Minister Adam Vojtech (for YES).

But large vaccination centers will no longer do this. They are gradually closing. The last thousand people vaccinated Český Krumlov in the sports hall on Thursday. They don’t have enough applicants for the next days.

“The space is for eight hundred to a thousand people a day. It can be done in smaller quantities at the outpatient clinic. So we close and go to the hospital, “says the manager of the vaccination center of the Český Krumlov Hospital, Karla Kolouchová.

Specifically, the vaccination will be moved to the pediatric outpatient clinic. Vaccines will start vaccinating here in early August. “In the surgery, we will provide approximately 250 patients with vaccine every day,” describes the head nurse of the Český Krumlov children’s ward.

It is the hospitals and also general practitioners who should be in charge of a possible third dose. According to the minister, the distribution of smaller packages of Pfizer vaccine for surgeries is already planned.

Vaccination advice

Coronaviruses can also be fully infected. This is confirmed by studies from Singapore or Israel. However, vaccines mainly protect against the severe and complicated course of the disease, when patients have to go to the hospital. And the vaccinee is also less likely to infect someone else.

Most often, the disease breaks out within 14 days of vaccination. This applies to the first and second dose. Data from the Czech Republic also show this. However, only a fraction of those vaccinated, about four thousand, became infected in this way. This is one of the reasons why a person is considered infection-free after vaccination only two weeks after the second dose.

Under current rules, fully vaccinated people do not need to be quarantined even in high-risk contact with the infected person. And they also avoid testing and quarantine when traveling from high-risk countries. But some experts criticize this, precisely because they can also become infected.

“I definitely think that people who have a two-dose schedule should go on holiday, the single-dose scheme turned out to be insufficient, and I think it’s the alpha and omega, plus testing people who have returned from vacation,” says the epidemiologist and former Minister of Health Roman Prymula.

At the moment, an average of 20,000 people a day come to the first dose of the vaccine in the Czech Republic. The speed in the area of the first doses rather gradually slows down. Compared to Europe, the vaccination rate is slightly below average. In the European Union, almost 57 percent of people already have at least the first dose. In our country it is 51 percent.

Disputes continue over whether to go to vaccination after suffering from the disease. The ministry canceled the three-month period, which did not work. Now it is recommended to be vaccinated immediately after the end of the isolation. But according to some experts, this is not good.

“There is simply an increased risk of side effects after vaccination with high levels of antibodies, there are studies. Not that you would die for it, but it is useless, it is useless to be vaccinated at a time when a person has antibodies, “says Jiřina Bartůňková, head of the Department of Immunology of the Second Faculty of Medicine and Motol University Hospital.

Vaccination of health professionals

According to Prime Minister Andrej Babiš (YES), there will be no mandatory vaccination against coronavirus in the Czech Republic, even for healthcare professionals. However, Minister of Health Vojtěch did not rule it out in the future. About a quarter of medical staff do not yet have the vaccine.

Zuzana Drábová, a doctor at the Department of Infectious Diseases at the Bulovka University Hospital, now mainly has patients with other diseases. But last spring she received the first ones with coronavirus.

“They came to my service, so then I was anxiously awaiting the result, whether they came out positive or negative,” says the doctor. Even though she was protecting herself, she considered the risk of infection. “I was worried not about myself, but that I could infect my family, so I was glad when it was vaccinated that we could protect ourselves,” adds Drábová.

However, some health professionals still do not have vaccinations. These are almost 17 percent of doctors or 25 percent of nurses. The reasons why they did not get the vaccine are various, some do not want to, others, for example, have suffered from the disease and are waiting because they still have antibodies.
