If you receive Social Security benefits and live in Colombia, you may need to complete a Survivor Form
If you receive Social Security benefits, live in Colombia, and the last two digits of your Social Security Number range from 50 to 99, the Social Security Administration (SSA) mailed you a Survivor Form (FEQ) between June and July 2023. If you did not return this first form, a second form was sent to you on October 17, 2023. Both included instructions for returning the form in a provided envelope, to the SSA processing center in Wilkes-Barre, PA.
You must submit your completed form to SSA by mid-December 2023; failure to do so may result in the suspension of your benefits in the following year. If you are 90 years of age or older and your benefits are paid through a representative, you must also complete a Survivor Form, regardless of the terminal of the last two digits of your Social Security Number.
Do not submit a duplicate Survival Form (FEQ) if you already submitted one during the second half of 2023. If you have not received a form, you must take the following actions:
Complete the form SSA-7162 (Pdf-59Kb) if you: receive your own payments.
- Complete the form SSA-7161 (Pdf-74Kb) if you: receive payments through a representative or you are a representative.
- Make sure to include:
- Social Security number
- Name
- Address
- Signature (note: we cannot accept digital or electronic signatures)
- Date
The Social Security Administration cannot accept this form by email or fax. If you have not yet submitted your form, you must send the original document (no copies) to the following address:

The Federal Benefits Unit in Santo Domingo cannot confirm whether your form was received and will not respond to your requests for confirmation. You will need to verify the delivery status with the tracking number provided by your shipping company.
If you are 90 years old or younger, receive your own benefits, and your Social Security Number ends in 00 to 49, you do not have to submit a Survivor Form during 2023. A form will be sent to you between June and July 2024. If you meet the requirements for a Survival Form but your official address is in the United States, you can ignore this message.
