How to treat torn, scratched wounds and scars?

[이데일리 이순용 기자]A, a man in his 20s, got scratched when he fell downhill while riding a bicycle. I was worried that there would be a scar on my face for a lifetime, but after receiving the scar treatment, I got better clean and relaxed. I think that the wounds that are common around are relatively minor, but if not properly treated, they can leave severe scars, so be careful. Find out how to take initial response and treatment for each wound with the help of So-young Kim, professor of plastic surgery at Inje University, Sanggye Paik Hospital.

Common trauma-related diseases include lacerations (simple tearing), abrasions (shattering or peeling of the shallow skin layer), and loss of the entire skin layer.

◇’Hemostasis’ is the priority for bleeding, and gently press with clean gauze or tissue paper… Risk of infection if touched with hands or mouth

When trauma occurs, bleeding usually occurs. If you have bleeding, the first thing to do is stop bleeding. If you don’t have clean sterile gauze or gauze, use a clean tissue or cloth to gently press the bleeding area. Most of the bleeding due to capillary damage is stopped with pressure of 5 to 10 minutes, but the area where the bleeding has been temporarily stopped may bleed again because the wound is not completely healed.

Therefore, it should be noted that removing the gauze covered on the wound to check whether hemostasis can induce rebleeding by barely stimulating the hemostasis condition. It is better to stop the bleeding together than to remove the flesh that is about to fall off or stubbornly. Once the bleeding has ended, you should no longer touch the wound with your hand or suck the wound with your mouth. This is because there are numerous invisible bacteria and viruses in the hands or mouth, which increases the risk of wound infection or inflammation. In the case of bleeding from the heartbeat, there is a high possibility of arterial bleeding, and this may not stop even with pressure for more than 10 minutes, so you should visit the emergency room immediately.

◇If bleeding has stopped, disinfect and attach foam dressing… Helps regenerate wounds

Pour disinfectant or physiological saline onto the wound to remove foreign substances, and apply a little pressure to cover the wound with a foam dressing (polyurethane foam like a thin sponge). Foam dressings are effective because they not only absorb exudate or blood, but also reduce pain when removing disinfectant materials.

◇If you visit the hospital, wrap the wound with sterile gauze and you can transplant it.

In addition, if the flesh is cut off by a knife or a sharp object, it is helpful to wrap the fallen tissue in sterile gauze if possible and visit the hospital. It is necessary to check the condition of the fallen skin first, but in some cases, it is possible to transplant using the fallen skin tissue without additional skin transplantation to cover the defective area.

◇Wounds must be treated by a specialist from the beginning… It affects how you leave scars afterwards

Professor Kim So-young said, “It is important to visit the hospital because the doctor must directly see the trauma area and check whether it is possible with only disinfectant treatment, whether sutures are required, whether there are foreign substances to be removed, and whether there are any accompanying fractures, etc.” In addition, “if a foreign substance such as dirt or asphalt enters the wound, it must be removed before the wound heals, but these foreign substances may go deeper and deeper as time passes, resulting in an unsightly scar accompanied by a traumatic tattoo.” “The wound is deep and sutured If you need this, you can go to the hospital within 24 hours and have a suture surgery.”

The average period for removing the stitches after suturing takes about 4-5 days for the face, 7 days for the chest and stomach, and 10-14 days for the arms and legs and back, but it may vary depending on the condition of the wound. In particular, in the case of abrasion around the torn wound, the healing period varies depending on the peeling of the skin layer, that is, the depth. The extent of the scar remaining depends on the depth, direction, location of the wound, the degree of contamination, and the characteristics of the skin each individual has. Therefore, it is most important to manage it from the beginning.

Professor Kim said, “It is important to continuously manage scar treatment from the beginning. Depending on the type of scar, silicone ointment, silicone band, suture tape, steroid injection, early laser treatment, etc. are used in parallel to reduce the worsening of scars in bad directions and achieve the best scar. He emphasized that it is more important than anything else to start treatment from the initial stage of trauma and receive continuous management from scar treatment and care, rather than thinking that it will be delayed anyway because the scar remains.

Skin transplantation using flesh tissue that has fallen off the entire skin. (If the flesh has fallen off, it is helpful to visit the hospital by wrapping tissue in sterile gauze.)
