REQUISITION FOR REGISTRATION OF THE RENEWAL OF THE ADVERTISING OF A RIGHT
The rights whose publication is likely to be renewed are those whose publication has been limited by an extreme effective date in the initial registration request. This is the case for mortgages, restrictions on the right to dispose of and fixed-term rights. The renewal of the registration of a mortgage is done using the form RH-Requisition for registration of a movable mortgage. The renewal of the publication of other rights is done using the RG form General requisition for registration.

MANDATORY INFORMATION
In section 1, enter “advertising renewal”.
2. Last effective date of registration:
A new effective date of the advertisement must be indicated in figures in the form yyyy-mm-dd (e.g.: 2009-08-31). Unless there are exceptions, the registration of the right will be automatically canceled the day after the effective date.
The parts which must be designated in the requisition are the same as those of the right for which renewal is requested. For example, if it concerns the renewal of the publication of the rights resulting from a lease, the landlord and the tenant are the required parties as for the registration of the rights resulting from the lease.
If there are more than two parties or if one party is acting by representation, use Annex AP. If a party is acting under a name other than its own and you want this assumed name to be revealed in the registration, use Annex AD.
For each of the designated parties, indicate their status as holder, settlor or other by checking box “a”, “b” or “c” as applicable in sections 4 and 12 of the form or in section 1 of annexes AP and AD . When box “c” is checked, the quality must be specified (see section 3.1.1.2).
Rights status request:
You can request a statement of rights registered under a person’s name in the registration application upon payment of the related fees.
To receive the certified statement of rights registered under the name of a person, check the box provided for this purpose (“certified statement of rights”) under the designation of the party concerned; if you want more than one certified statement of rights registered under the same name, indicate the number in the box instead.
25. Reference to the registration referred to in the register of personal and movable real rights:
Reference must be made to the registration for which renewal is required by mentioning the registration number of the right.

The notice must be signed by the person requesting registration (see section 3.1.1.5).
INFORMATION FACULTATIVE
3. and 11. Address notice number:
Registration of the address for notification purposes may be required by entering the beneficiary’s address notification number in the space provided for this purpose. If annex AP or AD is used, complete section 2 or 5 as appropriate.
If the notification address has already been registered with respect to the right covered by the renewal, the registration of the address remains and there is no need to renew it; in this case, do not enter anything under sections 3 and 11 of the form and 2 and 5 of annexes AP and AD respectively.
