As for the first two requirements, a self-certification should suffice, which will eventually be subjected to random checks. For the scrapping, however, you can take the old TV directly to the physical retailer who will deal with the matter, or to an authorized recycling center by obtaining a scrapping certificate that you can present to access the bonus.
