Heartland and Bourbon Tick-Borne Viruses Spread Across US
Two potentially fatal tick-borne pathogens, the Heartland virus and the Bourbon virus, are expanding their geographic footprint across the United States, raising concerns among public health researchers regarding surveillance gaps and underdiagnosed human infections. Transmitted primarily by the lone star tick, these pathogens have now been identified in at least nine states following recent surveillance data from the University of Nebraska, according to scientific reports published in August. While severe disease manifestations remain rare among the general population, the northward and westward migration of the tick vector—driven by environmental shifts and reforestation—presents emerging diagnostic hurdles for clinicians.
Commercial diagnostic tests for these specific pathogens do not currently exist, requiring specialized laboratory blood sample analysis for confirmation. Public health researchers emphasize that early symptoms closely mirror other vector-borne illnesses, creating a significant risk of underdiagnosis in non-endemic regions.
Geographic Expansion and Surveillance Data
The lone star tick, historically localized across the southeastern United States, has steadily expanded its range into northern and western territories. Data analyzed by researchers at the University of Nebraska, encompassing over 7,000 ticks collected between 2022 and 2026, confirmed the presence of both the Heartland and Bourbon viruses in Sarpy County, while the Heartland virus was additionally identified in Richardson County. This development establishes Nebraska as the ninth state where these pathogens have been detected in vector populations. Although no human cases have been formally diagnosed within Nebraska, Joseph Fauver, a researcher at the University of Nebraska, noted a real possibility of undetected human transmission given the overlapping clinical presentations with other vector-borne diseases.
The geographic spread is not limited to the Midwest. In New York, public health officials recently identified the state’s first human case of Bourbon virus infection. An individual on Long Island contracted the pathogen following multiple tick bites, highlighting the expanding exposure risks outside historical endemic zones. Luis Marcos, a researcher tracking the spread, indicated that the Bourbon virus is likely more prevalent than current clinical metrics suggest, largely due to limited routine screening protocols.
Pathology and Clinical Presentation
Both pathogens present diagnostic challenges because their acute symptoms overlap heavily with other tick-borne illnesses or other viruses. Infected patients frequently present with acute fever, fatigue, headaches, anorexia, myalgia, arthralgia, and nausea. While the vast majority of infections resolve without severe complications or are asymptomatic, a small proportion of patients develop severe, life-threatening clinical manifestations requiring hospitalization.

Since its initial identification in 2012, the Heartland virus has accounted for more than 80 confirmed human cases across the central and eastern United States, resulting in at least four documented fatalities, including a fatal case involving a 32-year-old hiker. Similarly, the Bourbon virus, discovered in 2015, has been linked to multiple deaths since its initial characterization. Because commercial assay kits are unavailable for either virus, clinical confirmation relies almost exclusively on sending patient blood samples to specialized reference laboratories or public health agencies for advanced molecular testing.
Addressing Diagnostic and Clinical Gaps
The emergence of these viral agents in new geographic regions underscores the necessity for heightened vigilance among primary care physicians and infectious disease specialists. When patients present with acute febrile illness following outdoor exposure in areas where lone star ticks are newly established, standard panels may yield negative results for more common local pathogens. For individuals experiencing persistent, unexplained systemic symptoms or severe post-infection complications, seeking specialized clinical evaluation is critical.
