As part of a leave replacement, we are looking for General Practitioners m/f for a full-time or part-time CDD contract, at the HAD
of the Notre Dame Polyclinic in Draguignan.
Proposed dates: 14
days in July (your choice) and from August 12 to 21
Your missions:
Within a multidisciplinary team
you participate in the development of the patient’s care plan and you
ensure the quality of medical care. You intervene at
patients’ homes and coordinate care with the attending physician
and medical specialists
Medical mission:
Evaluate and validate the admission of a patient in HAH.
Develop and readjust therapeutic projects, care plans in
collaboration with multidisciplinary teams
Make home visits and intervene in collaboration with the doctor
treating
Telephone on-call night and weekend (1 week out of 2)
Medical-administrative mission:
Pronounce the medical opinion on the admission of patients in HAH
Realize the production of medical information
Participate in weekly multidisciplinary meetings for reassessment of
patients
Participate in the palliative care staff

Participate in the development,
validation and evaluation of care protocols
Establish output CR
