The health insurance card, which also contains the tax code issued by the Revenue Agency, is used every time the citizen goes to the doctor, buys a medicine in a pharmacy, books an examination in an analysis laboratory, benefits from a specialist visit to the hospital and to the ASL or when using spa treatments and, in any case, every time they have to certify their tax code. The card, which is strictly personal, allows you to obtain health services even in the countries of the European Union. It is issued to all citizens who are entitled to health care by the National Health Service. 4

How can I request a duplicate of the health card?
To request a duplicate, in addition to the tax code or, alternatively, the complete personal data of the applicant, the reason for the request must be specified: theft or loss, or technical replacement if the card is damaged or illegible; in the latter case, the identification number of the TS or the tax code card to be replaced is requested. For security reasons, it is also necessary to enter some data that can be deduced from the return presented in the previous year: type of return (730, Unico, none), method of presentation and total income declared.

How and where will I receive the duplicate?
After sending the request, the data entered are compared with those registered in the Tax Registry. If the information is correct, the Card is sent to the address in the Tax Register. If the applicant is not assisted by the National Health Service, the tax code card is sent.
The service tells me that the duplicate cannot be issued, why?
The duplicate cannot be issued if:
- * the entered tax code is incorrect or not present in the Tax Register
- * the personal data entered are not present in the Tax Register
- * the person for which the duplicate is requested has died in the Tax Register
- * the tax code entered or calculated on the basis of the personal data indicated has been updated in another tax code
- * the subject is associated with an insignificant address in the Tax Register
- * the identification number of the Health Card or the tax code card entered is incorrect (in case of request for technical replacement)
- * for the subject, the issuance of a card is already in progress, eg. for a duplicate requested by an ASL or an agency office, or the automatic reissue of the expiring card is in progress
- * the subject is the holder of a Health Card / National Services Card.
