Erasmus MC reports this on 19 July. With the new counter, the hospital can focus more on care-related prevention. “That means that we make a healthy lifestyle part of regular treatment,” says Van Rossum. The counter can also offer support with lifestyle care or social help at home.
Lifestyle treatments
In the lifestyle care counter, lifestyle care coordinators receive patients from the departments of Internal Medicine, Cardiology and Liver Diseases. The coordinators carry out an intake and measurements and then, together with the patient, look for a lifestyle treatment that suits their wishes and needs, such as an exercise program, combined lifestyle intervention (GLI), a diet, walking coaching or physiotherapy – but also references to the social domain, for example for debt counseling, are possible. The lifestyle care coordinators work together with a network of primary care providers.
VGZ
One of the partners in the project within Erasmus MC is health insurer VGZ. If the lifestyle care counter is successful, it will be available to all patients within Erasmus MC and possibly to other UMCs to create a network of lifestyle care providers with national coverage.
