Effectiveness of Combined Er:YAG and Long-pulsed Nd:YAG Laser Treatment for Stubborn Warts – Prospective Randomized Study

The combination of Er:YAG and long-pulsed Nd:YAG lasers in the same session is more effective than the Er:YAG laser alone in treating stubborn warts. Researchers of this prospective randomized study therefore recommend combining these modalities – if available.

Viral warts are a common skin disease caused by the human papillomavirus (HPV). In recent years, laser has been used more often to treat stubborn warts, with varying success rates. The aim of this study was to prospectively evaluate the effectiveness of combined treatment with the Er:YAG laser followed by the long-pulsed Nd:YAG laser compared to the Er:YAG laser alone for the treatment of persistent warts after 1 session.

The study included 240 lesions from 24 patients (12 men, age: 12-72 years) treated between May and September 2022 after unsuccessful topical treatment and cryotherapy. The number of warts per patient ranged from 2 to 48, with a median of 6 and a mean of 10. 120 lesions underwent the combination treatment of the Er:YAG and long-pulsed Nd:YAG laser; the remaining 120 lesions were treated with the Er:YAG laser alone. After 5 weeks, the effectiveness of the treatment was assessed and classified: complete response (100%), partial response (50-99%) and poor response (< 50%).

The clearance rate in the group treated with both lasers was statistically significantly higher than that in the group treated with the Er:YAG laser alone (p = 0.008). The complete response rate was 48% (58 of 120 warts) in the combination group and 29% (35 of 120 warts) in the Er:YAG group.
