Early tracheostomy saves patient’s life with Covid-19

An early tracheostomy saved her life from Alejandro, a 56-year-old patient who was discharged on July 9, Specialty Hospital of the Mexican Institute of Social Monitoring (IMSS) in Jalisco, after receiving medical attention for two months for the complications caused by Covid-19.

The leader of the team of specialist doctors who attended the case, Pedro Ramírez Barba, said that given the patient’s conditions, which required intubation, the probability of death was more than 90 percent.

His overweight limited his respiratory capacity, even with ventilatory support, to high parameters, so he opted to perform an early tracheostomy, a pioneering intervention in the world, since the time of its performance has not been established and is generally practiced around day 21 mechanical ventilation.

We call it early because it takes place between the 7th and the 14th day instead of the 21st, as is normally stated. Thanks to this, sedation could be removed from the patient and his lung improvement is increasing. The patient is diabetic and hypertensive, and developed acute kidney failure that also required continuous hemodialysis, so the risk of death was very high, but he is already in the process of rehabilitation, “he explained.

The patient not only recovered and was discharged, but it is predicted that the consequences will be minimal and he will be able to return to his activities, said Dr. Ramírez Barba.

The specialist attributed this achievement to the multidisciplinary work of Internal Medicine, Pulmonology, Angiology, Hematology, Nephrology, Otorhinolaryngology and NursingBecause from each branch, the patient’s case was evaluated and decisions were made to improve the state of health.

This is a very important success story because not only did he survive, he was one of the most serious patients we have ever had. Many maneuvers had to be carried out, but fortunately the contributions made by the experts working in this retraining hospital allowed for a positive result, ”he said.

He considered that this can be applied in similar cases to save more lives, “the performance of an early tracheostomy, after clinical evaluation, is not established anywhere in the world,” he said.
