In the British city of Birmingham, surgeons by mistake killed two twins in the womb. The doctors wanted to save one of the babies who could have survived, but they confused them. As a result, both babies died.

Fiona Reynolds, head physician at the Birmingham Women’s and Children’s Health Foundation, called the incident a “fatal mistake”:
“Following this tragic incident, a full and comprehensive investigation was immediately carried out and the results were communicated to the family along with our sincere apologies and condolences.”
