Device failed – woman suffered uterine damage

An unsuccessful uterine operation, due to faulty technical equipment, ended with a patient in Värmland having to be treated in a life-threatening condition.

The incident is now reported to the Inspectorate for Care and Care (Ivo) according to lex Maria, writes region Värmland on its website.

It was in September 2022 that a woman with bleeding problems came into the healthcare system to undergo a so-called endometrial ablation, reduction of the lining of the uterus.

But the operation got off to a bad start, when a technical instrument malfunctioned and indicated technical failure. The device was replaced with another one, which initially worked well – until it too warned of a technical error.

The operation had to be canceled and after being examined with ultrasound, the patient was sent home.

But after two days the woman was back in the hospital with an affected general condition. It was only when she was then admitted for surgery that it turned out that she had suffered damage to the intestine and that the uterus was perforated. The woman, whose condition was life-threatening, had to be treated in hospital for two weeks afterwards.

The healthcare system has also made a report to the Swedish Medicines Agency and sent the instrument for analysis by the manufacturer. Operations with the device have been halted until further notice.
