Paperwork and threats from patients’ relatives deny candidates
There are fewer and fewer candidates to head diagnostic and consultation centers in Plovdiv. Family doctors with specialties who earn up to BGN 10,000 per month refuse to participate in the competitions.
“Interest is low, and this year the contracts of most heads of health facilities are expiring,” said Dr. Kalin Kalinov, director of municipal health care in Plovdiv.

He makes the reservation that not every jeep earns so much, but in any case the income from practicing medicine is much higher than the salary for a boss position. And the headaches are not so much if you just treat patients without doing administrative work.
“My salary as a manager is BGN 2,500. If I work in two offices, I will get much more “, admitted the head of the DCC in Plovdiv. And he confirmed that family doctors with extensive practices bring in 6-7 thousand levs per month. And those who do medical research get even more. Thus, their salaries really reached BGN 10,000.
The large staff shortage of doctors with specialties was also on the heads of managers. “One of my radiologists is on long leave, I don’t have a second doctor and that’s a problem. Somewhere the steam leaked, elsewhere the electricity went out. You run, you argue “, admits another manager, who is waiting for his contract to expire and to sit in the doctor’s office.
This was the main reason why there were no candidates for bosses. “You are not only involved in medical activities. I constantly feel guilty if a doctor raises a patient’s tone. On the other hand, the relatives of the sick themselves are constantly pouring their anger on us, threatening us. “Someone fainted and they didn’t help him, according to relatives, and then run to give explanations,” said a manager.
He describes his work as ungrateful. “You do your best, and there are always those who are dissatisfied.” Apart from the quarrels that came to him more, the paperwork was also great.

They maintain heating in the building, security, manage over 100 people – nurses, nurses. There is a lot of work, ”says Dr. Rositsa Popova, who is a family doctor at DCC-1.
However, she clarifies that her remuneration is far from what people are talking about. Her patient list included 1,200 people, 250 of whom were uninsured. Her expenses were over BGN 3,000 – a nurse’s salary, insurance, rent, ambulance fee, consumables.
But Popova does not want to become a manager either. “I was once asked to apply. I said I love my job as a doctor, I don’t want to be an administrator. All my life I have been used to examining and treating. Younger people are needed for boss work. Administrative activity is not for everyone “, summarizes Dr. Popova. He believes that a salary of BGN 2,500 – 3,000 for a manager is not small at all.
Dr. Kalin Kalinov: In time, there may not be anyone willing to become a manager

– How many managers of the DCCs in Plovdiv have their contracts expired this year? and you have to run competitions for them, Dr. Kalinov?
– To most. And from now on I am worried whether there will be candidates. Interest has dropped significantly. Doctors who have a specialty and work in it are better paid. And they do not want to head commercial companies, such as medical institutions. Nowadays, this is associated with many problems, not just the pandemic. The personnel crisis affects everyone. It is a cascade of problems. There are no people at the moment. And the main source of pre-hospital care are family doctors. See what’s going on with them. In some DCCs, jeeps are already a quarter of the tenants, which was not the case 20 years ago.
– Why was there such a large influx of DCC managers in the past, and now you have to look for them in the houses and beg them?
– At that time, the legal successors of the polyclinics were the DCCs. All full-time positions in them were filled with specialists. There was no staffing problem then. They worked and were profitable companies. Now it is very difficult to get a plus DCC. And when a job is accompanied by solving difficult tasks, some of them unsolvable, very rarely a manager can agree to take over such a company.
– The current manager of the DCC in Plovdiv says that if he works in two offices, he will receive a higher salary than now.
“That’s right. And when he’s out of office, he turns the key while the manager is financially responsible.”
– Why are there only one candidate in the competitions for the head of the DCC lately?
– The time is not far off when there will not be a single candidate. Earlier, 10 people competed for one place for a trainee. At the moment there is one for two places, not her. There are no isolated processes in healthcare.
“Shouldn’t you go begging them at some point?”
– It is possible that in the future there will be no candidates for heads of DCC.
– As the director of the municipal health care, are you worried about this fact?
– A lot. And we need to move towards structural reform. We are currently building an alliance to provide scholarships and incentives for young doctors so that we can count on them tomorrow.
