Quick pads also in surgeries gods general practitioners and gods free choice pediatricians dell’Emilia Romagna. The measure, operational in a few days and which will last – except for further extensions – until as of December 31, 2020, is the result of a twofold agreement signed today of the Region: with Fimmg (for general practitioners) and with Fimp and Cipe-Sispe-Sinspe Federation (for free choice pediatricians).
The “raw material” is already there: almost 45mila rapid antigenic swabs, supplied to Emilia-Romagna since Commissioner for the Covid-19 emergency together with Personal protective equipment, have already been distributed in the warehouses of the individual Local Health Units and will very soon be delivered to doctors and paediatricians.
The agreements signed by the Region implement national collective agreements of 30 October 2020, which contain provisions for the enhancement of the services provided by medicine and general pediatrics in the activity of epidemiological investigation through diagnostic assessment. With a specific goal: help identify outbreaks quickly and isolate cases.
Quick swabs from general practitioners: how it works
According to the agreement signed, patients will have to book after having done a telephone triage to carry out quick antigenic swabs. They will be able to take the test by their general practitioner – if you work in your own office – i close asymptomatic contacts identified by the doctor himself or reported by the Public Health Department awaiting a quick swab; and suspected cases of contact that the general practitioner has to visit and who decides to have a quick test.
If the doctor works in structures made available by the health authority (for example, in a health center), he can be asked to expand his target also to clients of other general practitioners, specifically a close asymptomatic contacts after 10 days of isolation identified on the basis of a list sent by the Department of Public Health / Hygiene and Prevention to the identified doctor.
I doctors holding temporary posts, temporary, short-term or long-term replacement (including doctors who attend the specific training course), perform and report the activities related to the tests in the same way as the owners.
In the structures made available by the Usl Company do diagnostic tests, if necessary, also i continuity of care doctors (former medical guard), doctors of territorial health emergency (for example, in 118), doctors of service medicine (such as those who work in prisons) and doctors engaged in planned territorial activities.
Offices and premises
Local Healthcare Companies provide their support by identifying and organizing suitable locations where to carry out rapid tests if primary care doctors cannot perform them in their own office. A delivery mode may also be used drive through, according to a criterion of proximity to the catchment area, with methods defined at company level through specific agreements.
Supply of tests and Personal Protective Equipment (Dpi)
The supply of rapid antigenic swabs, or other tests provided for in the national collective agreement of 30/10/2020, is ensured to doctors by the Commissioner for the Covid-19 emergency together with personal protective equipment (masks, gloves, visors and gowns). All the material (tests and devices) has already been supplied to the logistics centers indicated by the Region, and from there to the warehouses of the Local Health Units. The methods of delivery to doctors are established at the company level. Doctors who do not perform swabs in the office will in any case be assured of the necessary Personal Protective Equipment.
Quick swab outcome management
In case of successful, the doctor communicates this to the patient and informs him about the path to follow. The doctor authenticates himself through his own file duly integrated or through the regional portal SOLE, in order to register the positive result; this also in order to report the case to the Department of Public Health. The doctor then assesses the patient’s clinical condition and informs him of the obligation to respect home isolation, or to continue his quarantine status pending the outcome of the confirmation (molecular) swab. The doctor therefore takes charge of the patient and provides him with the appropriate indications to follow. In case of negative outcome, the result is recorded on the regional portal SOLE and a certificate is issued to the patient.
Taking charge of patients
General practitioners take charge of their patients placed in isolation or quarantine – with the possible support of the Special Care Continuity Unit (Usca) – providing the interested parties with the hygienic-sanitary and behavioral information to follow. They evaluate the opportunity to place subjects who need more support, specifically patients at risk of complications, under active surveillance, with telephone or electronic contact.
Quick swabs from free choice pediatricians
Pediatricians of free choice use rapid antigenic swabs during outpatient or home practice in favor of its clients. Whether you work in your own office (even for assisted by other pediatricians) or if you work in facilities made available by the Usl Company, the pediatrician performs antigen tests by appointment and after having done a telephone triage priority for i close asymptomatic contacts – on the first useful date after the expiry of the isolation – identified on the basis of a list sent by Department of Public Health to the doctor identified (it is the privileged target); for the close asymptomatic contacts identified by the pediatrician; for i suspected cases of contact that the pediatrician visits and decides to have a quick test.
Also in this case, as for general practitioners, the Local Health Authorities provide their support preferably by identifying suitable locations where to carry out rapid tests if free choice pediatricians are unable to perform them in their own office. A delivery method may be used drive through, according to a criterion of proximity to the catchment area. In consideration of the possible difficulties in carrying out tampons in the pediatric age group, the Local Health Authorities will provide support staff as needed.
Supply of tests and Personal Protective Equipment (Dpi), as for general practitioners, they are paid by the Commissioner for the Covid-19 emergency.
Quick swab outcome management
In case of successful, the pediatrician communicates the outcome to the patient’s parents or family members, informing them of the path to follow. The pediatrician authenticates himself through his own folder appropriately integrated or through the regional portal SOLE to record the outcome, evaluates the clinical conditions and strongly recommends isolation at home pending the outcome of the confirmation swab, when required. The pediatrician takes charge of the patient and provides the appropriate hygienic-sanitary and behavioral indications to be followed during the period in question. In the event that the pediatrician carries out a swab on a patient who is not among the choices in charge, he will communicate the outcome to the child’s pediatrician and the latter will take charge of his patient.
In case of negative outcome, the result is recorded on the regional portal SOLE and a certificate is issued to the patient.
Taking charge of patients
Pediatricians of free choice take charge of their patients placed in isolation or quarantine, with the possible support of the Special Care Continuity Unit (Usca), providing the families concerned with the hygienic-sanitary and behavioral information to follow. They evaluate the opportunity to actively supervise, with daily telephone or electronic contact, children / young people who need greater support, specifically those at greater risk of complications.
