Your solution to get vaccinated is MyLVHN
As an essential worker, you are eligible for the COVID-19 vaccine earlier than the general public. We are currently vaccinating health care workers and residents and employees of skilled nursing facilities and long-term care facilities for the elderly. We will also vaccinate other essential workers in the future, according to the Pennsylvania Department of Health phased vaccination plan. If you open a MyLVHN account, you will be notified when it is your turn to make an appointment (see instructions below)
If you got vaccinated against another disease in the last 14 days, you should delay your COVID-19 vaccination. You should not get any other vaccines 14 days before or after the COVID-19 vaccine.
All essential workers will need to show their company identification card at the time of the appointment. If your ID does not include a photo, you will be required to show a government-issued photo ID as well. If your company does not use ID cards, simply bring a letter printed on official company paper that includes your position and shows a government-issued photo ID.
Am I an essential worker?
If you are an essential worker as defined by the Pennsylvania Department of Health and the COVID-19 Response Guide for Critical Infrastructure Workers from the Cybersecurity and Infrastructure Security Agency, then you can follow the three steps below to pre-register for the vaccine.
Steps to pre-register for the vaccine
- Login to your MyLVHN account
The easiest and fastest way to pre-register is using MyLVHN, LVHN’s patient portal. People who do not have a MyLVHN account can open one at MyLVHN.org. You do not need to be an LVHN, LVPG, or Coordinated Health patient to open a MyLVHN account.Do you need help opening a MyLVHN account? Watch the video below or call 888-402-LVHN (5846).
- Click on “Menu”
- Complete a COVID-19 Questionnaire for Essential Workers. If your employer is not on the list, select “Not Listed”.
After completing the questionnaire, you will be notified when you can select the date and place to vaccinate in MyLVHN.
How does LVHN determine who is eligible for the COVID-19 vaccine?
LVHN is committed to vaccinating all members of our community as soon as possible and will cover all applicable costs. LVHN’s vaccination plan against COVID-19 follows the same Pennsylvania Department of Health phased method of vaccination.
