COVID-19 Update: 4/4/2022 – Fallsburg Central School District

Dear Parent/Guardian:

Today we learned that a staff member at BCES tested positive for Covid-19 and is required to isolate. There were no exposures as a result and no students or staff are required to quarantine. There are no Covid-19 updates for the Junior Senior High School at this time. Both buildings will remain open.

Please be sure to check your messages frequently for any important updates from the school district.

Anytime students are quarantined, they will participate in their full class schedules remotely during the quarantine period.  Quarantined students are expected to be fully engaged and attendance counts for them the same as it would if they were in the school building.

Test Reporting Portal: To help the public more easily obtain needed documentation for COVID-19 isolation and quarantine affirmation/release forms, Sullivan County Public Health Services has a new test reporting portal for public use. The forms can be found at www.sullivanny.us/Departments/Publichealth/Coronavirus/Attestation.

Isolation Affirmation Form: For those who have tested positive for COVID-19, an Isolation Affirmation Form is required and can be obtained via email by reporting test results to Public Health on the portal.  www.sullivanny.us/Departments/Publichealth/Coronavirus/Attestation.

Quarantine Affirmation: For those who have been exposed to someone with COVID-19, a Quarantine Affirmation Form is required and can be filled out, downloaded and printed from the portal. https://sullivanny.us/sites/default/files/departments/PHS/Coronavirus/QuarantineAffirmationFILLABLE.pdf


Additional Help: Those who are unable to use or access these forms can call Public Health at 845-513-2268 for assistance.  All forms should be submitted to the school nurses.
