Complete Revascularization: Reduced Risk of Death and Recurrence in Elderly Heart Attack Patients

When an elderly person has a heart attack, a study has found that revascularization surgery, which opens not only the problematic blood vessels but also the narrowed arteries, reduces the risk of death and the risk of heart attack or stroke.

Because elderly heart attack patients have other medical conditions and can be frail, doctors often leave only the offending vessel open, leaving other partially blocked vessels intact. However, according to a research team at the University of Ferrara Hospital in Italy, removing all blocked blood vessels lowered the risk of death, having another heart attack or stroke, and reducing the need for other surgery.

In a clinical trial on 1445 male and female patients over the age of 75, the research team performed a procedure that opened only the clogged heart arteries or all of the main clogged arteries. The results showed that 9% of patients who opened all of their narrowed arteries died within a year or had another heart attack, compared to 14% of patients who opened only one problematic artery.

In addition, 16% of patients who opened all of the narrowed arteries were at risk of death, another heart attack or stroke, or needed another procedure, but 21% of those who opened only one blocked artery.

Dr. Simone Viscaglia (cardiology) of the research team said, “Doctors worry about complications, so the standard of care for elderly patients is to treat only the affected area.” It has been shown that complete revascularization is better for elderly patients with multivascular disease or who have developed vascular disease.”

Other experts agree that treating all severely clogged arteries should be the standard, regardless of the patient’s age. Dr. Greg Furnarow, director of the Ahmanson-UCLA Cardiomyopathy Center, USA, said, “This clinical trial shows that complete revascularization to treat all major coronary artery occlusions in patients with heart failure can be performed. “It has proven to be superior to focusing only on the artery in question.”

The results of this study (Complete or Culprit-Only PCI in Older Patients with Myocardial Infarction) were published in the international journal ‘New England Journal of Medicine’.
