Cardiovascular Risk Factors in Primary Sjögren’s Syndrome (pSS): 20-Year Study Findings

The aim of this retrospective study was to assess the prevalence of cardiovascular events and risk factors in patients with pSS over a 20-year period (2000-2022). The influence of glandular/extraglandular involvement and anti-Ro/SSA and/or anti-La/SSB autoantibodies was examined.

A total of 102 pSS patients were included; 82% were women, the mean age was 65 (± 24) years and the mean disease duration 12.5 (± 6) years. 36 patients (36%) had at least 1 cardiovascular risk factor. Arterial hypertension was diagnosed in 59% of patients, dyslipidemia in 27%, diabetes in 15%, obesity in 22% and hyperuricemia in 18%. A history of arrhythmias was found in 25%, conduction disorder in 10%, peripheral arterial disease in 7%, venous thrombosis in 10%, coronary artery disease in 24% and cerebrovascular disease in 22% of patients. Patients with extraglandular involvement had a higher prevalence of arterial hypertension (p = 0.04), dyslipidemia (p = 0.003), high LDL levels (p = 0.038), hyperuricemia (p = 0.03), and coronary artery disease (p = 0.01) after adjustment for age, gender, disease duration and the significant variables in the univariate analysis. Patients with Ro/SSA and La/SSB autoantibodies had an increased risk of hyperuricemia (p = 0.01), arrhythmia (p = 0.01), coronary artery disease (p = 0.02), cerebrovascular disease ( p = 0.02) and venous thrombosis (p = 0.03). In multivariate logistic regression analysis, a higher risk of cardiovascular risk factors was associated with extraglandular involvement (p = 0.02), corticosteroid treatment (p = 0.02), ESSDAI > 13 (p = 0.02), inflammatory markers including high erythrocyte sedimentation rate (p = 0.007) and serological markers such as low C3 (p = 0.03) and hypergammaglobulinemia (p = 0.02).
