Turkish Gynecology and Obstetrics Association (TJOD) held its 20th National Congress in TRNC. 1,800 doctors, including experts working in the earthquake area, attended the congress held between 17-21 May.

Dr. A press conference was held with Volkan Kurtaran. The details of the meeting, where innovations, problems in the field of gynecology and the newest methods in IVF treatment were discussed:

‘EXOSOMES EGGS MAKES YOU YOUNG’

Stating that the “exosome” method has been added to the developments that accelerated with PRP in IVF treatment and supported by stem cell studies, TJOD President Prof. Dr. Bülent Tıraş pointed out that the method in question is one of the effective solutions for female infertility. “This method, which is one of the new generation cellular treatments, is a method that rejuvenates the eggs in women with low ovarian reserve and therefore increases the chance of IVF. Tıraş explained the method as follows: Exosomes are small vesicles of nano size. Communication agents are structures we call nanovesicles. All of them allow cells to communicate with each other. Just like we communicate by letter or e-mail… These were not easily obtained from blood. However, Utkan Demirci from Stanford University in the USA enabled them to be obtained with the method he developed. With the new method, we can extract exosomes from the blood and filter them. We have been using this method for about 4 months. Initial results are good. In summary, PRP, stem cell and exosome method in women has also become a new opportunity to increase the chance of maternity for women who have low egg reserve, have reached a certain age or, on the contrary, have gone through menopause at a young age.

‘CAESAREAN RATIOS NOT ACTUALLY HIGH’

Pointing out that the issue of normal birth or cesarean section is always discussed, TJOD Secretary General of the Board of Directors Op. Dr. Volkan Kurtaran stated that the issue was also brought to the agenda in the congress and made the following statement: It seems that cesarean section rates in our country are high, but in fact it is not. The numbers are high as our documenting possibilities have increased. Since the pregnant women are followed better, the cesarean rate seems to be high, that is, more than 50 percent. However, the rate did not increase. If the mother and baby are good in every way, we prefer normal delivery. If there is the slightest negativity, we want a cesarean section. It is not correct to hold it as a normal birth. These views coincide with the Ministry of Health. Also, if the doctor is afraid of malpractice lawsuits, he turns to cesarean section. If a doctor is free, if there are laws protecting him, he will give birth naturally. Kurtaran also noted that while 1 million 200 thousand births occur annually in our country with a population of 85 million; He also drew attention to the fact that 5 million refugees gave birth to 750-800 thousand.

‘GYNECOLOGICAL SURGERY NOW BOTH PRACTICAL AND TRACELESS’

Stating that ‘natural opening surgery’, that is, vaginal surgery, which is a practical and traceless solution to common problems in women such as uterine prolapse, uterine fibroids, and cysts, is one of the most important innovations in the last period, TJOD Board Treasurer Prof. Dr. Gazi Yıldırım gave the following information about this method: Finally, we used to perform some operations with an incision from the navel. However, there is no incision in the new method, so there is no scar. Because we perform the surgeries vaginally. The patient is discharged in one day and recovers in 5-6 days.

‘MOTHER DEATHS ARE DECREASINGLY DECREASING’

Stating that great success has been achieved in preventing maternal deaths in the last 15-20 years in Turkey, TJOD Vice President Prof. Dr. İsmail Mete İtil continued his words as follows: The maternal mortality rate, which was 100 per 100 thousand in the past, has decreased to 13 per 100 thousand today. With this result, we have caught the civilized countries. The ultimate goal is to reduce this to less than 10 per 100 thousand. This is a great achievement in a population-dense country like Türkiye. One of the most important reasons behind this is that there are well-trained obstetricians and gynecologists in our country. It is also an increase in cesarean sections. Home birth is no more. Among the causes of maternal deaths are problems such as embolism, high blood pressure and heart attack. Therefore, it is not possible to prevent some complications. On the other hand, the problem of urinary incontinence, which is seen in one of every 4 women, increases with age. Uterine prolapse also causes this problem. In treatment, patch surgeries began to be replaced with natural tissues. Vaginal surgeries came to the fore.
