Buckingham Palace announced earlier this week that the King with Charles has been diagnosed with cancer. The issue occurred during treatment for an enlarged prostate, but according to Buckingham Palace, it is not prostate cancer. So far, the public has not been told which form of cancer the king has been diagnosed with.

Iltalehti asked Finnish urologists what is usually done in treatments related to prostate hyperplasia, and what could possibly occur in the treatments.

Urologists do not comment on King Charles’s treatment or cancer diagnosis, but talk about prostate hyperplasia and related procedures only on a general level.

Senior physician at the urology department in Hus Jukka Sairanen tells Iltalehti by e-mail that extensive examinations are not done due to prostate hyperplasia.

– In connection with the treatment of prostate hyperplasia, a bladder scan is performed. That is, if the patient had changes in the bladder, they would appear during the treatment, Sairanen writes.

A change found in the bladder could possibly mean bladder cancer. According to the Health Library, people between the ages of 75 and 79 typically get bladder cancer, and 75 percent of those affected are men. According to the Health Library, the prognosis for bladder cancer is relatively good.

Sairanen says that routine imaging or blood tests, where a tumor outside the prostate could be found, are not routinely performed.

– Of course, royals can routinely have different health checks, which we know nothing about, adds Sairanen.

Since there is no information available about the additional tests that may be performed on the royals, it is also not possible to draw any conclusions about King Charles’ form of cancer.

Prostate hyperplasia symptoms and treatment

Specialist in urology at Terveystalo, docent Mika Raitanen tells what kind of symptoms are common in patients suffering from benign prostatic hyperplasia.

– Patients with obesity have difficulty starting to urinate, intermittent or weak showers, and may have an increased need to urinate and night visits. These are the classic symptoms.

According to the treatment, the enlarged prostate constricts the urethra, which makes it difficult for urine to pass.

According to Raitanen, the primary form of treatment for an enlarged prostate is medication, but if the patient already suffers from very difficult symptoms from the start or if medical treatment does not provide sufficient help, then a surgical procedure is performed on the patient.

– A small prostate gland can be split open, where the tissue is not removed, but split open. The patient can also undergo prostate planing, where using electrical energy, the tissue is mechanically planed away with a metal loop in order to open up the urine passageway.

Raitanen says that in addition to the measures mentioned above, the tissue can alternatively be vaporized with a green laser or atrophied with water vapor. For a very large prostate, open surgery may be necessary.

– If the prostate gland is, for example, more than 100 grams, then open surgery can be a reasonable option. In open surgery, the capsule structure is left in place, but through the opening, the enlarged inner part of the prostate is removed from the wound and the capsule is closed. This differs from surgery for prostate cancer in that the capsule is also removed in surgery for cancer.

Because surgeries involve some risks and can cause some problems with ejaculation, for example, medication is usually tried before guidance for surgery. Raitanen says that the vast majority of hypertrophy patients get enough help from prostate shrinking and/or relaxing drugs.

Possible side effects are individual and depend on the medicinal product. Raitanen says that the possible harms of the medicines are always discussed with the doctor and the most suitable preparation is always chosen with the patient.

When to see a doctor

Terveystalo’s specialist in urology, docent Mika Raitanen explains what symptoms you should see a doctor for urgently.

1. If an elderly man starts having urinary incontinence, and especially if the incontinence starts at night. According to Raitanen, this can be caused by an overfilling of the bladder, and is alarming.

2. If the color of the urine becomes red or bloody.

– Especially if the urine of a smoker turns red to the eye, you need to seek treatment urgently. In this case, hematuria is alarming and the bladder should be checked, Raitanen says.

3. The cause of all urinary symptoms affecting the quality of life should be found out and treated. Raitanen recommends seeing a doctor in this case as well.

4. If urinating is painful, you should always find out the cause.
