After Nepal’s Yeti Airlines Flight 691 crashed, it didn’t take long for investigators to determine that pilot error was the cause of the crash. Namely, the plane’s captain, Anju Hativada, accidentally pulled the crank that turns the plane’s propeller blades parallel to the air flow, thereby significantly reducing power and causing the plane to hover. Instead, the fenders should have been raised (English flaps). As a result, the engines lost power, the plane lost speed and crashed. Neither the pilots nor the passengers had any idea that anything had gone wrong until the last moment.
