Since last week, the emergency department of the CHRU has had to deal daily with a greater influx of elderly patients from their homes. An unexpected situation that impacts an already weakened hospital system during this summer period.

Faced with this observation, the CPTS of the Nancy metropolis (territorial community of health professionals), the PTA of Greater Nancy (Gerard Cuny network – territorial support platform) and the HADAN (home hospitalization of the Nancy conurbation) , the mobile geriatric liaison team and the social service of the University Hospital of Nancy mobilized to set up, in record time, agile coordination in short circuits to find innovative solutions adapted to the situation of each patient.
Streamline the patient journey
Thus, from July 25, 2022, a city/hospital regulation cell will meet every 48 hours to monitor the system and quickly identify solutions adapted to the condition of each patient. This unique and temporary initiative aims to streamline the journey of patients over 75 years of age upstream but also upon arrival at the emergency room by using the region’s resources as proportionately as possible.
