On the table in her clinic in the city of Culiacan, doctor Rafaela Martinez Terrazas has a pile of applications from potential clients—women who want surgery. Most of them requested procedures related to what is known as “narcotics aesthetics”.

A woman with this hyper-feminist and exaggerated silhouette is often referred to as “la buchona” in Mexico—especially if she likes flashy designer things and has a boyfriend narco (member of a drug cartel).
“The average age of my patients is between 30 and 40 years. But very often younger women come—even minors under 18,” the doctor said.
“They compete with each other—who has the best body, or the smallest waist.”
Women and teenagers may come to consult with their mothers or friends. Others come with a man, or alone.
“A lot of times they come in with boyfriends paying for the surgery. And I’ve had various guys call me and say, ‘Hey, doctor – I’m going to send you a girl for surgery.’
