The health questionnaire includes specific questions about your state of health and in no way refers to the intimate aspects of your private life.
Depending on your choice, you can send your completed questionnaire in a sealed envelope or by a secure (tele) transmission procedure to the attention of the insurer’s medical adviser. If he needs additional medical information, he will get closer to you.
If you have already undergone during the last 6 months, within the framework of a previous insurance application, the medical examinations that the insurer needs, you can send the results of these examinations to the insurer’s medical adviser.
