To receive the COVID-19 vaccine, you must fill out the form to receive orientation and coordinate the appointment. Once you make your appointment, you will receive an email confirmation.
IMPORTANT:
- Ensure that you comply with the vaccination phase defined by the Department of Health at that time.
* If you do not meet that requirement, we will not be able to proceed with your appointment request. -

- Disclaimer Download
- Completed Vaccination Consent Download
- Fact Sheet Download
