The people of Prague owe its creation to Emperor Joseph II and his successor on the throne Leopold II, as the inscription on the facade still attests today. The rules relating to the construction of general hospitals were published by Joseph II in 1781. The first of them were created in Vienna (1784), Brno (1786) and Olomouc (1787).

When it opened, the hospital had some 300 beds and accommodated all patients, regardless of their state of health, religious affiliation or nationality. Provided that they had lived in the city for at least ten years and were able to present a certificate of poverty, the penniless people of Prague benefited from free care. First and second category rooms also existed for patients who had the means to pay for their care.
The hospital can also boast a long tradition of academic medicine, while clinical education has been offered there from the start.
