It was revealed that there were 59,000 subscribers who were delinquent on their health insurance premiums for more than 120 months (10 years), and the number of subscribers whose arrears exceeded 100 million won was revealed to be 15.
Points are being raised that the National Health Insurance Corporation (National Health Insurance Corporation), which has management duties, should strengthen collection measures for large and habitual defaulters.
According to People Power Party lawmaker Kim Mi-ae’s office on the 16th, as of the end of June this year, a total of 961,000 local subscribers (households) were in arrears on health insurance premiums. The total amount of arrears is 1.5371 trillion won.
As of June this year, the number of delinquent borrowers has already exceeded last year’s figure of 949,000 delinquent households. The amount of arrears also exceeded the amount of arrears (KRW 1.5363 trillion) as of the end of last year (refer to graph).
In particular, there are 59,000 households with long-term delinquency of more than 120 months, and the total amount of arrears was found to be 319.9 billion won.
According to the status of local subscriber delinquency by period over the past 10 years, 59,000 households with 120 months or more, 104,000 households with 60 to 120 months, 218,000 households with 24 to 60 months, 252,000 households with 12 to 24 months, There are 324,000 households living for less than 6 to 12 months, for a total of 957,000 households.
The total amount of arrears due to 59,000 local subscribers who have not paid health insurance premiums for more than 120 months is 319.9 billion won. For less than 60 to 120 months, it is KRW 347.8 billion, for 24 to less than 60 months, KRW 396.2 billion, for 12 to 24 months, KRW 243.1 billion, and for 6 to 12 months, KRW 171.7 billion.
According to the status of health insurance premium arrears by amount over the past 10 years, there are about 15 households that have not paid more than 100 million won in health insurance premiums. The total arrears are 1.9 billion won. In the case of less than 5000 to 100 million won, there are approximately 344 households, and the arrears amount to 22.1 billion won. In the case of less than 10 to 50 million won, there are approximately 16,000 households, and the arrears amount to 264.9 billion won. There are approximately 49,000 households earning less than 5 to 10 million won, or 333 billion won in arrears. Those paying less than 5 million won account for the largest proportion, with about 891,000 households in arrears amounting to 856.9 billion won.
The problem of the corporation’s management of health insurance premium arrears is pointed out every year during the government audit, but the corporation has not been able to come up with a clear solution. If you do not pay by the payment deadline even after receiving a reminder letter for premiums, the Corporation collects the arrears with the approval of the Minister of Health and Welfare. However, the Corporation is of the opinion that there is no way if you do not have enough income to pay health insurance premiums or if you intentionally use a bank account in another name.
The Corporation collects arrears from subscribers who have defaulted on health insurance premiums more than six times. Even if you are in arrears, you can receive hospital treatment with support from health insurance during the benefit restriction period, just like a subscriber who pays health insurance premiums.
However, for delinquent delinquents, such as high-amount or habitual delinquent delinquents, insurance benefits are limited in advance (the entire amount must be borne by the taxpayer) at the stage of applying for treatment at a medical institution. Those who fall into the category of high-amount or habitual delinquent taxpayers are those whose personal information has been disclosed as high-amount or habitual delinquent taxpayers, or subscribers of their households, and subscribers of households with annual income exceeding 20 million won or assets exceeding 100 million won.
The Corporation is notifying delinquent taxpayers that the Corporation’s contributions incurred for delinquent taxpayers who received medical treatment through health insurance during the benefit restriction period will be recovered as unfair profits. However, the Corporation explained that the recovery rate for unfair profits is low. It was stated that specific figures on the recovery rate of ill-gotten profits could not be disclosed.
It is pointed out that the Corporation needs to encourage payment by households that have delinquent health insurance premiums and strengthen collection. This is because if you intentionally do not pay health insurance premiums, it is against equity with the people who pay health insurance premiums even if they have no income.

Kim said, “For the sake of fairness, we need to increase the collection rate for high-amount and habitual defaulters who have the ability to pay,” and added, “However, as health insurance is an issue directly related to public health, various support measures should be prepared to provide health insurance benefits to those in need, including low-income people.” “We need to ensure that this is maintained,” he emphasized.
